  1. [Parametrial infiltration of cervix carcinoma: diagnostic value of contrast-enhanced fat-suppressed T1-weighted SE sequences at 1.5 tesla].

    To determine whether contrast-enhanced and fat-suppressed sequences contribute to the MR imaging diagnosis of parametrial invasion. 21 patients with carcinoma of the cervix were prospectively examined with a phased-array coil and a 1.5T MR-scanner using the following sequences: transverse T2-weighted turbo spin echo (T2-TSE), T1-weighted spin echo (T1-SE) and fat suppressed T1-weighted SE sequences before and after Gd-DTPA. The sequences were evaluated separately for the presence of parametrial invasion. Image quality and diagnostic confidence were classified on a scale of 0-10 (nondiagnostic-excellent). Findings were compared to the results of the pathohistological examination. Sensitivity, specificity and diagnostic accuracy were highest for T2-TSE sequences (100%, 79% and 86%, respectively). Contrast-enhanced T1-SE sequences with fat-suppression (71%, 79%, and 76%) showed no improvement compared to T2-TSE. Unenhanced fat-suppressed T1-SE (100%, 30%, and 56%) and unenhanced T1-SE (100%, 7%, and 38%) as well as contrast-enhanced T1-SE (86%, 20%, and 47%) were significantly worse than T2-TSE. With similar image quality (p < 0.05) diagnostic confidence was higher on T2-TSE than on any of the other sequences (p < 0.001). Considering the cost-effectiveness of the examination, for the MR diagnosis of parametrial invasion the use of fat-suppressed contrast-enhanced sequences can be abandoned in favour of T2-weighted TSE sequences.

  2. Assessment of Parametrial Response by Growth Pattern in Patients With International Federation of Gynecology and Obstetrics Stage IIB and IIIB Cervical Cancer: Analysis of Patients From a Prospective, Multicenter Trial (EMBRACE)

    Purpose: To assess disease response along the parametrial space according to tumor morphology in patients with International Federation of Gynecology and Obstetrics (FIGO) stage IIB and IIIB cervical cancer at the time of image-guided adaptive brachytherapy. Methods and Materials: Patients with FIGO stage IIB and IIIB cervical cancer registered as of November 2013 in the EMBRACE study were evaluated. Tumors were stratified according to morphologic subtype on magnetic resonance imaging (expansive and infiltrative), and the characteristics of those subtypes were analyzed. Parametrial involvement at diagnosis and at brachytherapy was evaluated, and the response to chemo-radiotherapy was classified as good, moderate,more » or poor. The response grade was compared between the 2 groups and analyzed with regard to tumor volumes, and dosimetric parameters. Results: A total of 452 patients were evaluated, of whom 186 had expansive growth type and 266 had infiltrative morphology. Patients with infiltrative tumors had more extensive disease, as indicated by a higher rate of FIGO stage IIIB disease, as well as radiologic evidence of extension into the distal parametrial space and to the pelvic side wall on magnetic resonance imaging. Cervical necrosis was more common in the infiltrative group. Good response was more common in the expansive group (34% vs 24%; P=.02), and poor response was more common in the infiltrative group (11% and 19%; P=.02). Mean gross tumor volume at diagnosis was equal in both groups (51.7 cm{sup 3}). The high-risk clinical target volume was larger in infiltrative tumors (37.9 cm{sup 3} vs 33.3 cm{sup 3}, P=.005). The mean high-risk clinical target volume D{sub 90} was slightly higher in expansive tumors (92.7 Gy and 89.4 Gy, P<.001). Conclusion: Infiltrative tumors are more advanced at presentation and respond less favorably to chemo-radiotherapy when compared with expansive tumors that are more or less equivalent in size.     Hyperandrogenemia predisposes an organism toward developing impaired insulin sensitivity. The aim of our study was to evaluate endocrine and metabolic effects during early allostasis induced by a fructose-rich diet (FRD) in normal (control; CT) and neonatal-androgenized (testosterone propionate; TP) female adult rats. CT and TP rats were fed either a normal diet (ND) or an FRD for 3 weeks immediately before the day of study, which was at age 100 days. Energy intake, body weight (BW), parametrial (PM) fat characteristics, and endocrine/metabolic biomarkers were then evaluated. Daily energy intake was similar in CT and TP rats regardless of the differences in diet. When compared with CT-ND rats, the TP-ND rats were heavier, had larger PM fat, and were characterized by basal hypoadiponectinemia and enhanced plasma levels of non-esterified fatty acid (NEFA), plasminogen activator inhibitor-1 (PAI-1), and leptin. FRD-fed CT rats, when compared with CT-ND rats, had high plasma levels of NEFA, triglyceride (TG), PAI-1, leptin, and adiponectin. The TP-FRD rats, when compared with TP-ND rats, displayed enhanced leptinemia and triglyceridemia, and were hyperinsulinemic, with glucose intolerance. The PM fat taken from TP rats displayed increase in the size of adipocytes, decrease in adiponectin (protein/gene), and a greater abundance of the leptin gene. PM adipocyte response to insulin was impaired in CT-FRD, TP-ND, and TP-FRD rats. A very short duration of isocaloric FRD intake in TP rats induced severe metabolic dysfunction at the reproductive age. Our study supports the hypothesis that the early-androgenized female rat phenotype is highly susceptible to developing endocrine/metabolic dysfunction. In turn, these abnormalities enhance the risk of metabolic syndrome, obesity, type 2 diabetes, and cardiovascular disease.

  4. How often parametrial involvement leads to post-operative adjuvant treatment in locally advanced cervical cancer after neoadjuvant chemotherapy and type C radical hysterectomy?

    Parametrial involvement (PMI) is one of the most important factors influencing prognosis in locally advanced stage cervical cancer (LACC) patients. We aimed to evaluate PMI rate among LACC patients undergoing neoadjuvant chemotherapy (NACT), thus evaluating the utility of parametrectomy in tailor adjuvant treatments. Retrospective evaluation of consecutive 275 patients affected by LACC (IB2-IIB), undergoing NACT followed by type C/class III radical hysterectomy. Basic descriptive statistics, univariate and multivariate analyses were applied in order to identify factors predicting PMI. Survival outcomes were assessed using Kaplan-Meier and Cox models. PMI was detected in 37 (13%) patients: it was associated with vaginal involvement, lymph node positivity and both in 10 (4%), 5 (2%) and 12 (4%) patients, respectively; while PMI alone was observed in only 10 (4%) patients. Among this latter group, adjuvant treatment was delivered in 3 (1%) patients on the basis of pure PMI; while the remaining patients had other characteristics driving adjuvant treatment. Considering factors predicting PMI we observed that only suboptimal pathological responses (OR: 1.11; 95% CI: 1.01, 1.22) and vaginal involvement (OR: 1.29 (95%) CI: 1.17, 1.44) were independently associated with PMI. PMI did not correlate with survival (HR: 2.0; 95% CI: 0.82, 4.89); while clinical response to NACT (HR: 3.35; 95% CI: 1.59, 7.04), vaginal involvement (HR: 2.38; 95% CI: 1.12, 5.02) and lymph nodes positivity (HR: 3.47; 95% CI: 1.62, 7.41), independently correlated with worse survival outcomes. Our data suggest that PMI had a limited role on the choice to administer adjuvant treatment, thus supporting the potential embrace of less radical surgery in LACC patients undergoing NACT.   5. Clinical stage I carcinoma of the cervix. Value of MR imaging in determining invasion into the parametrium.

    Using MR imaging with a body coil parametrial invasion was determined prospectively in 169 consecutive patients considered on the basis of clinical examination to have carcinoma confined to the cervix. After radical hysterectomy correlation with histologic examination was performed for the left and right parametrium separately. The criterion for parametrial invasion was a high-signal-intensity lesion with disruption of the full thickness of the cervical stroma combined with areas of abnormal signal intensity within the parametrial region on T2-weighted images. Histologic examination showed that 18 parametria in 13 patients were invaded by tumor. MR had an overall accuracy of 93%, a sensitivity of 89%, and a specificity of 93% in demonstrating parametrial involvement. Positive and negative predictive values were 43% and 99%. The main weakness of MR was 21 false-positive tests. This represents a limitation when MR is performed with a body coil.

    We aimed to compare expression levels of antiapoptotic and proapoptotic genes in parametrial and vaginal tissues from postmenopausal women with and without pelvic organ prolapse (POP). We hypothesized that the expression of genes that induce apoptosis may be altered in vaginal and parametrial tissues in postmenopausal women with POP. Samples of vaginal and parametrial tissues were obtained from postmenopausal women with (n = 10) and without (n = 10) POP who underwent vaginal or abdominal hysterectomy. Expression levels of antiapoptotic (BCL-2, BCL-XL) and proapoptotic (BAX, BAD) genes were studied by real-time reverse-transcription polymerase chain reaction (RT-PCR). Gene expression levels of BCL-2 (P < 0.001), BCL-XL (P < 0.001), BAX (p = 0.001), and BAD (p = 0.004) were all higher in vaginal tissues from the POP group compared with the non-POP group. Similarly, gene expression levels of BCL-2 (p < 0.001), BCL-XL (p < 0.001), BAX (p < 0.001), and BAD (p < 0.001) in parametrial tissues were also significantly higher in the POP group compared with the non-POP group. Additionally, expression levels of BCL-2 (p = 0.05), BCL-XL (p < 0.05), BAX (p = 0.05), and BAD (p = 0.07) in the POP group were higher in parametrial tissue than in vaginal tissue samples. Antiapoptotic and proapoptotic gene expression levels differed significantly between postmenopausal women with and without POP. Bcl-2 family genes were overexpressed in the parametrium of patients with POP compared with vaginal tissue, suggesting that the processes responsible for POP have a greater effect on parametrial tissue than vaginal tissue during the development of POP.

    Purpose: To review the National Cancer Database (NCDB) to evaluate postoperative high-risk cervical cancer patients for factors associated with a benefit from chemoradiation therapy (CRT) over external beam radiation therapy alone (EBRT). Methods and Materials: The National Cancer Database was queried for women with cervical cancer treated with hysterectomy and adjuvant EBRT from 2002 to 2012. Only patients with pathologic lymph node involvement (LN+), positive surgical margins, and/or parametrial invasion were included in our analysis (on the basis of Peter's criteria). Univariable and multivariable analyses (MVA) were performed, and hazard ratios (HRs) with 95% confidence intervals (CIs) were calculated to investigatemore » for factors associated with of CRT utilization and overall survival (OS). Results: A total of 3053 patients met inclusion criteria, and 2479 received adjuvant CRT (81%), whereas 574 (19%) received EBRT alone. Factors associated with increased CRT utilization on MVA included age <69 years, year of diagnosis ≥2008, non-adenocarcinoma histology, and LN+. Use of CRT improved OS among the entire cohort on MVA (HR 0.76, CI 0.601-0.962; P=.022). On MVA, CRT improved OS in patients with LN+ as their sole Peter's criteria (HR 0.58, CI 0.413-0.814; P=.002). Chemoradiation therapy did not improve OS in patients with only positive margins (P=.73), only parametrial invasion (P=.95), or any combination of these 2 factors without LN+ (P=.63). Conclusions: The use of adjuvant CRT after hysterectomy improves OS in patients with high-risk cervical cancer compared with EBRT alone, but this benefit seems to be restricted to patients with LN+. The benefits of adjuvant CRT over EBRT alone in patients with parametrial invasion and/or positive margins (without nodal involvement) are unknown.« less

    Chronic pelvic pain (CPP) affects about 10-40% of women presenting to a physician, and is characterised by pain within the minor pelvis persisting for over 6 months. The Medline database was searched using the key words 'chronic pelvic pain' and 'pelvic congestion syndrome', published in English during the past 15 years. The condition markedly deteriorates the quality of life of the affected. Its aetiology has not been fully described and elucidated, although organic, functional and psychosomatic factors are implicated. Pain associated with parametrial varices was defined as pelvis congestion syndrome (PCS). Since the aetiology of CPP is complex, multi-directional diagnostic procedures are required. The main diagnostic methods employed are imaging examinations (ultrasound, computer tomography, magnetic resonance). Advances in interventional radiology considerably contributed to the CPP treatment. Currently, embolization of parametrial vessels is one of the most effective methods to relieve pain associated with pelvic congestion syndrome. Due to the complex aetiology of chronic pelvic pain, the most beneficial effects are obtained when the therapy is based on cooperation of the gynaecologist, physiotherapist, psychologist and interventional radiologist.

    Background: Because of the advancements in surgical techniques and laparoscopic instruments, total laparoscopic radical hysterectomy can now be performed for the treatment of uterine cervical carcinoma. We assessed the feasibility, complications, and survival rates of patients who underwent total laparoscopic radical hysterectomy with pelvic lymphadenectomy. Methods: We retrospectively collected data from the medical charts of 29 patients who had undergone surgery between 1998 and 2008. The following data were assessed: age, staging, histological type, number of lymph nodes retrieved, parametrial measures, operative time, length of hospital stay, surgical complications, and disease-free time. Results: The mean patient age was 37.07±10.45 years. Forty percent of the patients had previously undergone abdominal or pelvic surgeries. Mean operative time was 228.96±60.41 minutes, and mean retrieved lymph nodes was 16.9±8.12. All patients had free margins. No conversions to laparotomy were necessary. Median time until hospital dismissal was 6.5 days (range 3–38 days). Four patients had intraoperative complications: 2 lacerations of the rectum, 1 laceration of the bladder, and 1 lesion of the ureter. Three patients developed bladder or ureteral fistulas postoperatively that were successfully corrected surgically. Conclusion: Laparoscopic radical hysterectomy is feasible and has acceptable complications. The radicalism of the surgery must be considered, bearing in mind the parametrial measures and the number of lymph nodes retrieved. PMID:20202391

  11. [Carcinoma of the uterine cervical canal. Staging and biometric assessment with magnetic resonance].

    In uterine cervical canal carcinoma, the current clinical FIGO criteria often fail not only to differentiate stage IA2 from stage IB but also to demonstrate possible parametrial involvement. Moreover, the analysis of tumor volume and of the depth of neoplastic stromal invasion is not very reliable. The authors investigated MR accuracy in the definition of such variables: to this purpose, 24 patients with histologically confirmed endocervical adenocarcinoma were submitted to MRI, which was performed with an 0.5-T superconductive magnet. Sagittal and oblique transverse or sometimes coronal SE images, oriented so as to be perpendicular to longitudinal cervical major axis were obtained with T2 weighting (TR 1800 ms, TE 25-90 ms). MR data were correlated with pathologic findings. MR accuracy in demonstrating parametrial involvement was 92%, its sensitivity was 86% and specificity 97%. Volumetric MR data showed a high correlation (r = 0.970) with those derived from pathologic findings. In 92% of cases stromal invasion exceeded 5 mm. MRI, thanks to its high accuracy, should be included in diagnostic pretreatment protocols, even though FIGO criteria do not require it yet, especially in the presence of an endocervical adenocarcinoma. Moreover, the accurate definition of tumor volume can allow less extensive surgery with the same survival rates and fewer complications, which are frequently observed after radical hysterectomy.

    The Oncology Grand Rounds series is designed to place original reports published in the Journal into clinical context. A case presentation is followed by a description of diagnostic and management challenges, a review of the relevant literature, and a summary of the authors' suggested management approaches. The goal of this series is to help readers better understand how to apply the results of key studies, including those published in Journal of Clinical Oncology, to patients seen in their own clinical practice. A 55-year-old postmenopausal woman, gravida 5 para 5, with past medical history significant for hypertension, presented to the emergency department with profuse vaginal bleeding and a hemoglobin level of 9 g/dL. The biopsy from an irregular 6-cm cervical mass was consistent with moderately differentiated cervical squamous cell carcinoma. The physical examination did not reveal vaginal or parametrial extension of the tumor. Pelvic magnetic resonance imaging disclosed the known carcinoma, as well as a 9.2 × 7.7 × 6.7 cm anterior uterine fibroid (Fig 1). A staging positron emission tomography scan was negative for metastatic disease. After blood transfusion and vaginal packing, the patient was referred to discuss the immediate management of her newly diagnosed bleeding bulky cervical cancer. In the absence of parametrial or vaginal extension and in the absence of lymph node metastasis (both on clinical examination and imaging), she was classified as having International Federation of Gynecology and Obstetrics stage IB2 disease.

    To investigate the anti-obesity effects of escins extracted from the seeds of Aesculus turbinata BLUME, anti-obesity models in vitro and in vivo were employed. In a preliminary experiment, different solvent fractions of Aesculus turbinata BlUME as well as two isolated compounds were tested for their effects on pancreatic lipase (PL) in vitro. Subsequently, female ICR mice were fed a high fat diet with or without different concentrations of total escins for 11 weeks to examine body weight, parametrial adipose tissue weight, and hepatic triacylglycerol (TG) and total cholesterol (TC) contents. Plasma triacylglycerol levels (TG) after oral administration of lipid emulsions to rats were also investigated. The results showed that total escins (1 mg/ml) as well as two compounds isolated from total escins, namely escin Ib and IIa, showed inhibitory effects on PL activity. In vivo, total escins suppressed the increase in body weight, parametrial adipose tissue weight, TG content, and TC content in mice's liver; TG content in rat plasma was also reduced at 1, 2 and 3 h after oral administration of the lipid emulsion plus different concentrations of escins compared to those in the lipid emulsion groups. Meanwhile, mice fed a high fat diet plus 2% total escins for 3 d had an increased TG level in the feces compared to the HF group. The reason for this may be due to a delay in the intestinal absorption of dietary fat by inhibiting PL activity.

  14. Fertility conserving management of early cervical cancer: our experience of LLETZ and pelvic lymph node dissection.

    Presently, for those diagnosed with early cervical cancer who wish to conserve their fertility, there is the option of radical trachelectomy. Although successful, this procedure is associated with significant obstetric morbidity. The recurrence risk of early cervical cancer is low and in tumors measuring less than 2 cm; if the lymphatics are negative, the likelihood of parametrial involvement is less than 1%. Therefore, pelvic lymph nodes are a surrogate marker of parametrial involvement and radical excision of the parametrium can be omitted if they are negative. The aim of this study was to report our experience of the fertility conserving management of early cervical cancer with repeat large loop excision of the transformation zone and laparoscopic pelvic lymph node dissection. Between 2004 and 2011, a retrospective review of cases of early cervical cancer who had fertility conserving management within Glasgow Royal Infirmary was done. Forty-three patients underwent fertility conserving management of early cervical cancer. Forty were screen-detected cancers; 2 were stage IA1, 4 were stage IA2, and 37 were stage IB1. There were 2 central recurrences during the follow-up period. There have been 15 live children to 12 women and there are 4 ongoing pregnancies. To our knowledge, this is the largest case series described and confirms the low morbidity and mortality of this procedure. However, even within our highly select group, there have been 2 cases of central recurrent disease. We, therefore, are urging caution in the global adoption of this technique and would welcome a multicenter multinational randomized controlled trial.

  15. Effect of variation of trans-fatty acid in lactating rats' diet on lipoprotein lipase activity in mammary gland, liver, and adipose tissue.

    Lactation is associated with an increase in lipoprotein lipase (LPL) activity in the mammary gland (MG) and a decrease in adipose tissue because lactation redirects circulating substrates to the MG for milk synthesis. We investigated the effects of different dietary contents of trans-fatty acid (TFA) on LPL activity in maternal tissues and fatty acid composition in milk. Lactating rats were fed semisynthetic isocaloric diets containing 7% soy oil (control), 7% partially hydrogenated vegetable oil (7%-PHVO), 5% PHVO plus 2% soy oil (5%-PHVO), or 3.5% PHVO plus 3.5% soy oil (3.5%-PHVO). On lactation day 14, animals were decapitated and MG, liver, and parametrial adipose tissue were extracted to determine total lipid contents, percentages of TFA, and LPL activity. Milk lipid composition was examined by gas chromatographic analysis of the gastric content of 14-d-old suckling pups. Maternal consumption of TFA increased dietary TFA incorporation in MG and liver and decreased it in parametrial adipose tissue. Diets with higher trans concentrations (7%-PHVO) significantly increased lipid content in the MG, and all groups fed trans-based diets showed significant increases in LPL activity in the MG. Although LPL increased in the MG, milk of rats fed TFA-based diets had significant decreases in the percentage of essential fatty acids. TFA intake during lactation alters maternal lipid metabolism and the percentage of essential fatty acids in milk; therefore, it is important to alert the population to avoid excessive intake of TFAs during lactation.

    Chronic stress and persistently high glucocorticoid levels can induce brain atrophy. Corticotropin-releasing factor (CRF)-overexpressing (OE) mice are a genetic model of chronic stress with elevated brain CRF and plasma corticosterone levels and Cushing's syndrome. The brain structural alterations in the CRF-OE mice, however, are not well known. We found that adult male and female CRF-OE mice had significantly lower whole brain and cerebellum weights than their wild type (WT) littermates (347.7+/-3.6mg vs. 460.1+/-4.3mg and 36.3+/-0.8mg vs. 50.0+/-1.3mg, respectively) without sex-related difference. The epididymal/parametrial fat mass was significantly higher in CRF-OE mice. The brain weight was inversely correlated to epididymal/parametrial fat weight, but not to body weight. Computerized image analysis system in Nissl-stained brain sections of female mice showed that the anterior cingulate and sensorimotor cortexes of CRF-OE mice were significantly thinner, and the volumes of the hippocampus, hypothalamic paraventricular nucleus and amygdala were significantly reduced compared to WT, while the locus coeruleus showed a non-significant increase. Motor functions determined by beam crossing and gait analysis showed that CRF-OE mice took longer time and more steps to traverse a beam with more errors, and displayed reduced stride length compared to their WT littermates. These data show that CRF-OE mice display brain size reduction associated with alterations of motor coordination and an increase in visceral fat mass providing a novel animal model to study mechanisms involved in brain atrophy under conditions of sustained elevation of brain CRF and circulating glucocorticoid levels.     To determine the clinical and histological characteristics and prognostic factors of cervical cancer (CC) in young Peruvian patients. Retrospective analysis of patients younger than 35 years old diagnosed with CC between 2008 and 2012 in the Instituto Nacional de Enfermedades Neoplásicas. 449 patients had epithelial neoplasms. The main histological types were: squamous cell carcinoma (84.9%), adenocarcinoma (11.0%) and adenosquamous carcinoma (2.4%). The average tumor size was 4.98 cm. Anemia (55.7%), elevated creatinine (21.2%) and hydronephrosis (13.8%) were also identified. 82.3% of the patients presented locally advanced disease. Stages IIB (47.4%) and IIIB (25.8%) were the most common. Overall 5-year survival was 59.5% (I, 90.9%; II, 57.5%; III, 42.7% and IV, 13.3%). Elevated creatinine, anemia, tumor size, parametrial involvement and hydronephrosis were factors that affected survival. No significant relation was found between histological type and survival. The presence of anemia (adjusted hazard ratio [aHR]: 2.5; 95% confidence interval [CI 95%]: 1.6-4.0) and hydronephrosis (aHR: 1.6; CI 95%: 1.0-4.0) were independently associated with survival; likewise, the parametrial commitment with (aHR: 3.3; CI 95%: 1.5-7.2) or without (aRH: 2.6; CI 95%: 1.3-5.3) extension to the pelvic bone. Cervical cancer in young Peruvians is diagnosed in advanced stages. Overall survival in each stage is similar to the reported in older patients. The importance of conventional prognosis- related factors was confirmed. Anemia was an important independent prognostic factor requiring further investigations.

    Between January 1959 and December 1986, 10 of 328 patients (3%) treated with curative intent using primary radiation therapy for carcinoma of the cervix developed obstructive ureteropathy due to fibrosis. The mean age of the patients with obstructive ureteropathy was 45 years and the median time to obstruction was 26 months. The obstruction was unilateral in 8 cases and involved the parametrial portion of the ureter in at least 5 cases. No predisposing risk factor was found to be associated with the development of obstructive ureteropathy.     Uterine manipulators are a useful adjunct for robotic-assisted radical hysterectomy (RARH), but some surgeons avoid their use for fear of altering pathology or interpretation of lymphovascular space involvement (LVSI). We retrospectively compared clinico-pathological data and tumor pathology from patients with cervical cancer operated by laparotomy vs. RARH. Charts from cervical cancer patients who underwent radical hysterectomy from January-1997 to June-2010 were reviewed for tumor histology, grade, FIGO stage, lymph node status, LVSI, depth of invasion, and tumor size. A ConMed V-Care® uterine manipulator was used in all robotic cases. H&E stained slides from 20 robotic and 24 open stage IB1 cases with LVSI reported in the original pathology were re-reviewed by a blinded pathologist for analysis of tissue artifacts and LVSI. Two-hundred-thirty-six cases (185 open, 51 robotic) with stages IA2, IB1 and IB2 cervical cancer were reviewed. No significant differences in histology (squamous cell carcinoma, 65% vs. 51%; p=0.1), IB1 lesion size (≤2 cm, 62% vs. 61%, p>0.1), LVSI (34% vs. 39%, p>0.1), and depth of stromal invasion (p>0.1) was found between open and robotic groups. Histologic examination of all IB1 cervical carcinomas revealed a higher degree of surface disruption [45% (9/20) vs. 12.6% (3/24), p=0.038] and artifactual "parametrial carryover" [65% (13/20) vs. 29% (7/24), p=0.037] in robotic vs. open groups, respectively, but no significant differences in the rate of LVSI. RARH cases that utilized a uterine manipulator did not show any clinico-pathological differences in depth of invasion, LVSI, or parametrial involvement compared to open cases.     Purpose: We prospectively compared computed tomography (CT)– and magnetic resonance imaging (MRI)–based high-risk clinical target volume (HR-CTV) contours at the time of brachytherapy for cervical cancer in an effort to identify patients who might benefit most from MRI-based planning. Methods and Materials: Thirty-seven patients who had undergone a pretreatment diagnostic MRI scan were included in the analysis. We delineated the HR-CTV on the brachytherapy CT and brachytherapy MRI scans independently for each patient. We then calculated the absolute volumes for each HR-CTV and the Dice coefficient of similarity (DC, a measure of spatial agreement) for the HR-CTV contours. We identifiedmore » the clinical and tumor factors associated with (1) a discrepancy in volume between the CT HR-CTV and MRI HR-CTV contours; and (2) DC. The mean values were compared using 1-way analysis of variance or paired or unpaired t tests, as appropriate. Simple and multivariable linear regression analyses were used to model the effects of covariates on the outcomes. Results: Patients with International Federation of Gynecology and Obstetrics stage IB to IVA cervical cancer were treated with intracavitary brachytherapy using tandem and ovoid (n=33) or tandem and cylinder (n=4) applicators. The mean CT HR-CTV volume (44.1 cm{sup 3}) was larger than the mean MRI HR-CTV volume (35.1 cm{sup 3}; P<.0001, paired t test). On multivariable analysis, a higher body mass index (BMI) and tumor size ≥5 cm with parametrial invasion on the MRI scan at diagnosis were associated with an increased discrepancy in volume between the HR-CTV contours (P<.02 for both). In addition, the spatial agreement (as measured by DC) between the HR-CTV contours decreased with an increasing BMI (P=.013). Conclusions: We recommend MRI-based brachytherapy planning for patients with tumors >5 cm and parametrial invasion on MRI at diagnosis and for those with a high BMI.« less

    Cancer of the cervix uteri (CCU) ranks third in the incidence of malignancies in women. The choice of CCU treatment mainly depends on the extent of the tumor process, i.e., the stage of the disease. Determining the stage of CCU is based on the clinical classification of the International Federation of Gynecology and Obstetrics (FIGO) (2009) and has a number of substantial limitations in evaluating parametrial invasion, tumor spread to the pelvic wall, and involvement of regional lymph nodes and in determining the true tumor sizes. Magnetic resonance imaging (MRI) is now the method of choice in staging invasive CCU. Multiparameter MRI will be able to enhance the efficiency of diagnosing microinvasive CCU as well (FIGO 2009), to plan surgical and/or chemoradiation treatment, to evaluate its efficiency, and to diagnose locally recurrent CCU.

Questa tesi descrive i metodi utilizzati per caratterizzare il funzionamento di spettrometri ad immagine per l’esplorazione planetaria. L’attività svolta ha riguardato sia la misura in laboratorio dei parametri strumentali fondamentali sia l’analisi dei dati in volo su cui verificarne ed eventualmente correggerne la risposta. Il funzionamento degli strumenti e la metodologia di calibrazione vengono descritti nelle due parti principali e nelle appendici di questa tesi per gli esperimenti Cassini-VIMS-V e Rosetta-VIRTIS-M. La prima parte è dedicata all’esperimento VIMS-V sul satellite Cassini, il cui disegno ottico, originariamente proposto da Francis Reininger, verrà successivamente riutilizzato e perfezionato su VIRTIS-M (Reininger et al., 1994; Miller et al., 1996). Il capitolo 1 contiene una descrizione dei parametri ottici, strutturali, termici ed elettronici di VIMS-V mettendone in risalto gli aspetti più innovativi del progetto.     Na stężenie substancji toksycznych w dymie papierosowym w sposób oczywisty wpływać muszą warunki tlenowo-temperaturowe generowania dymu. Warunki te zależne są od sposobu palenia papierosa (topografii palenia). Topografię palenia charakteryzują następujące parametry: głębokość i szybkość zaciągania, czas przerw między zaciągnięciami i liczba zaciągnięć. W zdecydowanej większości dotychczasowych badań dotyczących składu dymu papierosowego, dym generowano w standardowych warunkach zalecanych przez Międzynarodową Organizację Normalizacyjną ISO i Federalną Komisję Handlu FTC. Obecnie coraz częściej kwestionuje się wykorzystanie standardowych metod laboratoryjnych, jako narzędzi do oceny dawek substancji toksycznych pobieranych przez palaczy w wyniku palenia Określenie rzeczywistych dawek substancji odtytoniowych na jakie narażeni są palacze określonej populacji wymaga zatem w pierwszej kolejności określenia topografii palenia palaczy należących do tej populacji. Celem pracy była kompleksowa charakterystyka parametrów topografii palenia papierosów populacji palaczy w Polsce i jej rozkładu statystycznego. Do badań zakwalifikowano reprezentatywną grupę 129 palaczy. Aby spełnić założenia reprezentatywności badanej grupy palących dokonano analizy struktury demograficznej palaczy papierosów w Polsce posługując się danymi GUS dotyczącymi ogólnej struktury ludności (płeć, profil wiekowy) oraz szczegółowej struktury populacji palaczy papierosów przeprowadzonych według kryteriów profilu wiekowego i liczby wypalanych papierosów. Pomiar indywidualnych parametrów topografii palenia przeprowadzono przy pomocy profesjonalnego przenośnego urządzenia CreSSmicro. Średnia objętość zaciągania się dymem zmierzona w badanej grupie wynosiła 60 ml, była, więc 78% wyższa od wartości normatywnej zalecanej do badań przez ISO (35 ml). Parametry z modeli lepkosprężystych są miarodajnymi wskaźnikami obrazującymi odporność mieszanek na deformacje. Można za ich pomocą prognozować głębokości koleiny w przyjętym modelu reologicznym [1]. W niniejszym artykule przedstawiono jaki wpływ na głębokość koleiny mają różne wartości parametrów reologicznych z analizowanego modelu lepkosprężystego oraz wpływ parametrów na kształt i przebieg krzywej pełzania. Przedstawione w artykule mieszanki mineralno - asfaltowe charakteryzują się zmiennymi parametrami reologicznymi, zatem trudno jest określić, który parametr decyduje o wielkości odkształcenia danej mieszanki. Mając na uwadze powyższe, w artykule podjęto próbę analizy zmiany wartości odkształcenia mieszanki mineralno - asfaltowej przy zmianie jednego oraz dwóch parametrów w danym modelu reologicznym - w tym przypadku - Bürgersa.

    RIASSUNTO Introduzione I parametri di pressione arteriosa, sistolica (PAS) o diastolica (PAD) hanno costituito I’oggetto di numerosi studi, alcuni dei quali stabiliscono un’associazione tra I’aumnto di tali parametri e la mortalità, in particolare alle età medie. I fattori di rischio di ipertensione più frequentemente studiati sono il consumo di alcol, il fumo e I’origine etnica in riferimento sia alle differenze genetiche che nella condotta alimentare, mentre i fattori di origine professionale sono stati presi in considerazione più raramente e in maniera poco approfondita, inoltre sono state riferite notevoli differenze nelle pressioni arteriose medie di individui che svolgono professioni diverse. Sembra infatti che i “colletti blu” siano più a rischio dei “colletti bianchi”. Obiettivi Il nostro scopo è valutare se determinati rischi o vincoli connessi con l’attività professionale o determinati stili di vita rappresentano un fattore di rischio per l’ipertensione arteriosa in considerazioni dell’aumentare dell’età. Elementi specifici che rendano conto di questa relazione non sono stati ancora completamente chiariti e soprattutto i dati sulla popolazione italiana sono molto limitati. Metodi La metodologia di ricerca utilizzata è stata quella dell’indagine francese ESTEV (Derriennic, Touranchet, Volkoff) - VISAT (Marquie, Janson), studio epidemiologico longitudinale su oltre 23000 lavoratori attivata in Francia nel 1990 al fine di indagare gli effetti a lungo termine delle condizioni di lavoro, riferite all’intera vita professionale, sia sull’invecchiamento sia sulla salute dei lavoratori. Analoga indagine è stata avviata in Italia, nel 2000, in 6 Regioni, prendendo in esame i lavoratori dipendenti di diversi settori produttivi appartenenti a 5 coorti di età: da 32 a 52 anni con intervallo di 5 anni.     We aimed to determine the sentinel lymph node detection rates, accuracy in predicting the status of lymph node metastasis, and if pathologic ultrastaging improves the detection of micrometastases and isolated tumor cells at the time of primary surgery for cervical cancer. A prospective, non-randomized study of women with early-stage (FIGO stage IA1 with lymphovascular space involvement--IIA) cervical carcinoma was conducted from June 2003 to August 2009. All patients underwent an intraoperative intracervical blue dye injection. Patients who underwent a preoperative lymphoscintigraphy received a 99m Tc sulfur colloid injection in addition. All patients underwent sentinel lymph node (SLN) identification followed by a complete pelvic node and parametrial dissection. SLN were evaluated using our institutional protocol that included pathologic ultrastaging. SLN mapping was successful in 77 (95%) of 81 patients. A total of 316 SLN were identified, with a median of 3 SLN per patient (range, 0-10 SLN). The majority (85%) of SLN were located at three main sites: the external iliac (35%); internal iliac (30%); and obturator (20%). Positive lymph nodes (LN) were identified in 26 (32%) patients, including 21 patients with positive SLN. Fifteen of 21 patients (71%) had SLN metastasis detected on routine processing. SLN ultrastaging detected metastasis in an additional 6/21 patients (29%). Two patients had grossly positive LN at exploration, and mapping was abandoned. Three of 26 (12%) patients had successful SLN mapping; however, the SLN failed to identify the metastatic LN. Of these 3 false negative cases, 2 patients had a metastatic parametrial node as the only positive LN with multiple negative pelvic nodes including negative SLN. One patient with stage IA1 disease and lymphovascular invasion had unilateral SLN mapping and a metastatic common iliac LN identified on completion lymphadenectomy of the contralateral side that did not map.   8. Laparoscopic en bloc resection of a para-cervical cancer with OHVIRA syndrome.

    There are some reports of cervical cancer with uterus didelphys but a case of clear cell carcinoma (CCCC) with Obstructed Hemivagina and Ipsilateral Renal Anomaly (OHVIRA) syndrome is extremely rare. The aim of this paper was to report a case of CCCC with OHVIRA syndrome and the difficulty in making a preoperative diagnosis. A 65 years old woman presented with postmenopausal bleeding and pelvic examination showed right paracervical mass. Preoperative confirmation of cervical carcinoma was difficult due to the location of the mass, which was inaccessible by cervical punch biopsy. Pelvic examination revealed a large mass in pelvic cavity without parametrial invasion and ultrasound showed approximately 70 mm cervical tumor. Laparoscopic surgery revealed clear cell carcinoma of the para-endocervix with OHVIRA syndrome. In the case of cervical carcinoma with OHVIRA syndrome, laparoscopic surgery is preferable for the diagnosis and management. Copyright © 2018. Published by Elsevier B.V.

    One of the most important operations in mining is blasting. Improper design of blasting pattern will cause technical and safety problems. Considering impact of results of blasting on next steps of mining, correct pattern selection needs a great cautiousness. In selecting of blasting pattern, technical, economical and safety aspects should be considered. Thus, most appropriate pattern selection can be defined as a Multi Attribute Decision Making (MADM) problem. Linear assignment method is one of the very applicable methods in decision making problems. In this paper, this method was used for the first time to evaluate blasting patterns in mine. In this ranking, safety and technical parameters have been considered to evaluate blasting patterns. Finally, blasting pattern with burden of 3.5 m, spacing of 4.5 m, stemming of 3.8 m and hole length of 12.1 m has been presented as the most suitable pattern obtained from linear assignment model for Sungun Copper Mine. Jedną z najpoważniejszych operacji wykonywanych w ramach prac wydobywczych są prace strzałowe. Niewłaściwe rozplanowanie prac powoduje problemy techniczne i stanowi zagrożenie dla bezpieczeństwa. Z uwagi na potencjalne skutki prac strzałowych i ich wpływ na kolejne etapy procesu wydobycia, właściwe rozplanowanie tych prac wymaga wielkiej uwagi i uwzględnienia kwestii technicznych, ekonomicznych a także bezpieczeństwa pracy. Dlatego też wybór najodpowiedniejszego schematu prowadzenia prac strzałowych zdefiniować można jako wieloatrybutowy problem decyzyjny (MADM - Multi Attribute Decision Making). Metoda przyporządkowania liniowego jest jedną z metod mających zastosowanie w rozwiązywaniu problemów decyzyjnych.     The purpose of the study was to evaluate the feasibility of using 99mTc-Al2O3- based radiopharmaceutical, a novel molecular imaging agent for sentinel lymph node detection in patients with invasive cervical cancer. The study included 23 cervical cancer patients (TlaNxMx- T2bNxMx) treated at the Tomsk Cancer Research Institute. At 18 hours before surgery, 80 MBq of the 99mTc-Al2O3 were injected peritumorally, followed by single-photon emission computed tomography (SPECT) of the pelvis and intraoperative SLN identification. Twenty-seven SLNs were detected by SPECT, and 34 SLNs were identified by intraoperative gamma probe. The total number of identified SLNs per patient ranged from 1 to 3(the mean number of SLNs was 1.4 per patient). The most common site for SLN detection was the external iliac region (57.2%), followed by the internal iliac, obturator, presacral and retrosacral regions (they amounted to 14%, respectively),and the parametrial region (1%). Sensitivity in detecting SLNs was 100% for intraoperative SLN identification and 79% for SPECT image.

    We compared the prognostic ability of the maximum standardized uptake value (SUVmax) and various biological marker expressions to predict recurrence in patients with surgically resected cervical cancer. A retrospective review identified 60 patients with cervical cancer who received [18F]fluorodeoxyglucose positron emission tomography/computed tomography (F-18 FDG PET/CT) at the time of the diagnosis of cancer. The SUVmax, expressions of carbonic anhydrase-IX (CA-IX), glucose transporter 1 (GLUT-1), and vascular endothelial growth factor (VEGF), and known prognostic factors were investigated. The median follow-up time was 22.2 months (range 3.4-43.1 months). Using univariate analyses, the stage (stage II, p = 0.0066), SUVmax (> 6, p = 0.027), parametrial involvement (p < 0.0001), and positivity for CA-IX (p = 0.0191) were associated with recurrences of cervical cancer. With the Cox proportional hazard regression model, the SUVmax was a potent predictor for disease-free survival (DFS). Although CA-IX expression was related to DFS in the current study, the potent predictor for DFS was SUVmax. Therefore, SUVmax is of greater prognostic value than biological marker expression in patients with surgically resected cervical cancer. © 2015 S. Karger GmbH, Freiburg.

    A careful macroscopic description with selection of representative tissue for histological examination is required for quality assurance, for assessing prognostic factors and for answering legal questions in (pre)cancerous lesions of the cervix uteri. Exact and standardized gross inspection and preparation are decisive for the quality of the histopathological statement. The extent of cervical carcinomas should be given in three dimensions, including the relative depth of invasion into the cervical wall. The report should include size, type (according to the WHO classification) and grading of the tumor, the presence of lymphatic as well as blood vessel invasion and perineural involvement. The statement for resection margins should include the vaginal, parametrial, rectal and vesical directions. It is also mandatory to document the number of lymph nodes with metastatic disease in relation to the total number of nodes investigated. The staging should follow the TNM system. In the handling of conisation specimens, it is important to appropriately document localization, horizontal expansion, depth of invasion including microinvasion of any dysplastic or malignant lesions. Clockwise dissection of the conisation specimen, total submission, and step sections are recommended. The preparation of exenteration specimens is a highly skilled job: the exact tumor dimension should be given in its relation to all resected organs and structures with special focus on resection margins.

    To investigate the implication of ribonucleotide reductase M2 (RRM2) in the carcinogenesis of uterine cervix and its relationship with clinicopathological characteristics and prognosis of cancer patients. The impact of RRM2 on cell viability was investigated in SiHa cervical cancer cells after RRM2 knockdown and the addition of cisplatin, which induces inter- and intra-strand DNA crosslinks. RRM2 immunoreactivity was evaluated by semi-quantitative H score among 29 normal, 30 low-grade dysplasia, 30 high-grade dysplasia and 103 invasive cancer tissue specimens of the uterine cervix, using tissue microarrays. RRM2 was then correlated with the clinicopathological variables of cervical cancer and patient survival. A greater toxic effect on cell viability using cisplatin was reflected by the greater reduction in RRM2 protein expression in SiHa cells. The RRM2 expression in cancer tissues was higher than that in high-grade dysplasia, low-grade dysplasia or normal cervical tissues. RRM2 upregulation was correlated with deep stromal invasion, large tumors and parametrial invasion and predicted poor survival. RRM2 is a new molecular marker for the diagnosis and clinical outcomes of cervical cancer. It is involved in cervical carcinogenesis and predicts poor survival, and may be a potential therapeutic target including in cisplatin treatment.

  16. Risk strata-based therapy and outcome in stage Ib-IIa carcinoma cervix: single-centre ten-year experience.

    To review the outcome of stage (Ib, IIa), cervical cancer patients were primarily treated with radical hysterectomy and risk-based postoperative therapy. Between January 2001 and December 2011, 601 cases underwent surgery followed by tailored therapy. Patients were classified into low risk (pelvic lymph node negative, tumour less than 4 cm, no evidence of lympho-vascular invasion, less than one-third of thickness of surgical stoma involved), intermediate risk (positive lympho-vascular space invasion, tumour size more than 4 cm, and deep invasion of cervical stroma), and high risk (pelvic lymph node involved, positive parametrial, or vaginal margins) groups. Postoperative adju-vant therapy in the form of radiotherapy alone to those with intermediate risk and chemo-radiotherapy to those with high risk was given to patients. The median follow-up was 60 months. The majority of patients had intermediate risk. The overall event-free survival (EFS) at five years was 74.37%, with EFS of 86.5% in those from the low-risk group, 73% in those from the intermediate-risk group, and 64% in those from the high-risk group. In conclusion, risk strata-based adjuvant postoperative therapy is able to provide a favourable outcome in patients with stage Ib-IIa cervical cancer with a nearly 11% improvement in survival compared with historical control.

    The research was conducted on the basis of four different types of hard coal and one type of brown coal. There are typical coals commonly used as fuel in Polish CFB boilers. The combustion process was conducted at a temperature of 850°C and the atmosphere of ambient air as well as in the mixture of oxygen and carbon dioxide in different proportions. The research was carried out using specially prepared cubical coal particles with measurements of 15×15mm and also 10×10 mm. The change of the mechanical properties was analyzed based on three parameters, i.e. compression strength, Vickers hardness and fracture toughness. The analysis was supplemented by microscopic images of the surface of the particles using an atomic force microscope. The results obtained clearly indicated the mechanical changes of the coal during its combustion, particularly at the moment of ignition of the char. Moreover, the results correlate very well with the processes of coal comminution that have been described by other authors (Basu, 1999; Chirone et al., 1991) during combustion in the circulating fluidized bed and also explain the sudden change of susceptibility to erosion in the conditions with and without combustion. The measured values can be used as strength parameters in the modelling of the mass loss of coal particles in conditions of circulating fluidized bed combustor that are hard to describe. Są to typowe węgle energetyczne wykorzystywane powszechnie jako paliwo w kotłach fluidyzacyjnych w Polsce.   18. SITES OF LIPOPROTEIN LIPASE ACTIVITY IN ADIPOSE TISSUE PERFUSED WITH CHYLOMICRONS

    Lipoprotein lipase activity was studied in rat parametrial adipose tissue perfused with chylomicrons and in gelatin blocks containing postheparin plasma and chylomicrons. The tissues and blocks were fixed in glutaraldehyde and incubated in 0.035 M CaCl2-0.1 M Tris medium (pH 8.3) at 38°C. The doubly labeled chylomicron triglycerides (glycerol-3H and palmitate-14C) in the tissues and blocks were hydrolyzed during incubation to free fatty acids (FFA) and the FFA remained in the specimens; hydrolysis was inhibited by 0.004 M diethyl paranitrophenyl phosphate (E-600). Incubated blocks and tissue were treated with 0.05 M Pb(NO3)2, postfixed in OsO4, dehydrated with acetone, embedded in Epon, and examined by electron microscopy. The incubated blocks contained electronlucent areas and granular and laminar precipitates at sites of hydrolysis. Similar precipitates were found in incubated tissue, within vacuoles and microvesicles of capillary endothelium, and in the subendothelial space (between the endothelium and pericytes), but not in the capillary lumen or in or near fat cells. The cytochemical reaction was greatly reduced, in blocks and tissues incubated with E-600. It is concluded that plasma glycerides are hydrolyzed by lipoprotein lipase in capillary endothelial cells and in the subendothelial space of adipose tissue and that glycerides across the endothelial cells within a membrane-bounded system.   19. Nerve-sparing radical hysterectomy for stage IA2-IIB cervical cancer: 5-year survival of 501 consecutive cases.

    The purpose of this study was to assess the 5-year survival and morbidity in cases with radical hysterectomy and pelvic lymphadenectomy with pre- and postoperative irradiation performed to treat Stage IA2-IIB cervical cancer. During a 10(1/2)-year period between July 1990 and December 2000, 501 consecutive radical hysterectomies with bilateral pelvic lymphadenectomy were performed by the same gynecological surgeon in Stage IA2, IB, IIA and IIB cervical cancer. The patients were treated by pre- and postoperative irradiation as well. Apart from recurrence, perioperative complications were minimal with no long-term morbidity. The absolute 5-year survival rates for the patients in Stage IA2, IB1, IB2, IIA and IIB were 94.4%, 90.7%, 84.1%, 71.1%, and 55.4%, respectively. The respective 5-year survival rates for patients without or with lymph node metastasis were 94.5% and 33.3% in Stage IB2, 81.7% and 48.7% in Stage IIA and 70.2% and 36.5% in Stage IIB, respectively. Nerve-sparing radical hysterectomy with pelvic lymph node dissection and pre- and postoperative irradiation remains the treatment of choice for most patients with early-stage and even Stage IIB cervical cancer. The radicalism and extent of lymph node dissection and parametrial resection should be individualized and tailored to tumor- and patient-related risk factors.

    New strategies to improve Ca absorption and bone health are needed to address the current state of osteoporosis prevention and management. Inulin-type fructans have shown great promise as a dietary intervention strategy, but have not yet been tested in a young female model. Our objective was to investigate the effect of long chain (LC) inulin on bone mineralization and density in growing, female rats, as well as the quality of growth. Weanling Sprague-Dawley rats were assigned to inulin or cellulose treatments for either 4 or 8 weeks. Growth was measured weekly and quality of growth assessed using fat pad weights and dual-energy X-ray absorptiometry (DXA). Whole body (WB) and selected regions were analysed for bone mineral density (BMD) and body composition by DXA. Serum markers of bone turnover were assessed by enzyme-linked immunosorbent assays. Ca and P concentrations were determined in excised femurs by inductively coupled plasma spectrometry. Feeding inulin resulted in 4 % higher femoral weight (adjusted for body weight) and 6 % less feed intake. Inulin did not affect WB or regional BMD, but was associated with a 28 % lower parametrial fat pad mass, 21 % less WB fat mass and 5 % less WB mass. In summary, LC-inulin lowered body fat mass, without consequence to bone density in growing female rats.

  1. Stereo x-ray photogrammetry applied for prevention of sigmoid-colon damage caused by radiation from intrauterine sources

    Radiation therapy of cervix carcinoma is applied in this Institute by means of modified Stockholm method in combination with external beam irradiation. In 1968, parametrial portals were replaced by large planeparallel opposed fields extending cranially to LIII/LIV with central shielding in order to avoid overdosage in the area of intracavitary treatment. This resulted in a marked increased incidence of serere sigmoid-colon radiation lesions from 0.25% to 4%; predominantly in Stage I and II patients. Therefore two measures have been introduced: beginning in 1972 measures were taken to prevent the cranial displacement of the uterus during intracavitary treatment in order tomore » avoid shortening the distance between the radioactive sources and the sigmoid-colon; from 1973 stereo X ray photogrammetry (SRM) was applied for dose determinations at points of the sigmoid-colon, which were seen to be located close to the applicator. When SRM data indicated that a high dose at the sigmoid-colon might occur, treatment modifications enabled prevention of radiation damage. Change of position of the applicator was the first to be considered. In the last seven years no surgical intervention had to be performed because of a sigmoid-colon lesion resulting from an unexpected high radiation dose delivered by intrauterine sources. The local recurrence rate was not increased following treatment modifications for prevention of sigmoid-colon radiation damage.« less

    Tributyltin chloride (TBT) is an environmental contaminant used in antifouling paints of boats. Endocrine disruptor effects of TBT are well established in animal models. However, the adverse effects on metabolism are less well understood. The toxicity of TBT in the white adipose tissue (WAT), liver and pancreas of female rats were assessed. Animals were divided into control and TBT (0.1 μg/kg/day) groups. TBT induced an increase in the body weight of the rats by the 15th day of oral exposure. The weight gain was associated with high parametrial (PR) and retroperitoneal (RP) WAT weights. TBT-treatment increased the adiposity, inflammation and expression of ERα and PPARγ proteins in both RP and PR WAT. In 3T3-L1 cells, estrogen treatment reduced lipid droplets accumulation, however increased the ERα protein expression. In contrast, TBT-treatment increased the lipid accumulation and reduced the ERα expression. WAT metabolic changes led to hepatic inflammation, lipid accumulation, increase of PPARγ and reduction of ERα protein expression. Accordingly, there were increases in the glucose tolerance and insulin sensitivity tests with increases in the number of pancreatic islets and insulin levels. These findings suggest that TBT leads to adiposity in WAT specifically, impairing the metabolic functions of the liver and pancreas. Copyright © 2015. Published by Elsevier Ireland Ltd.

    The effect of progesterone (P4) on fructose rich diet (FRD) intake-induced metabolic, endocrine and parametrial adipose tissue (PMAT) dysfunctions was studied in the adult female rat. Sixty day-old rats were i.m. treated with oil alone (control, CT) or containing P4 (12 mg/kg). Rats ate Purina chow-diet ad libitum throughout the entire experiment and, between 100 and 120 days of age drank ad libitum tap water alone (normal diet; CT-ND and P4-ND) or containing fructose (10% w/v; CT-FRD and P4-FRD). At age 120 days, animals were subjected to a glucose tolerance test or decapitated. Plasma concentrations of various biomarkers and PMAT gene abundance were monitored. P4-ND (vs. CT-ND) rats showed elevated circulating levels of lipids. CT-FRD rats displayed high (vs. CT-ND) plasma concentrations of lipids, leptin, adiponectin and plasminogen activator inhibitor-1 (PAI-1). Lipidemia and adiponectinemia were high (vs. P4-ND) in P4-FRD rats. Although P4 failed to prevent FRD-induced hyperleptinemia, it was fully protective on FRD-enhanced plasma PAI-1 levels. PMAT leptin and adiponectin mRNAs were high in CT-FRD and P4-FRD rats. While FRD enhanced PMAT PAI-1 mRNA abundance in CT rats, this effect was absent in P4 rats. Our study supports that a preceding P4-enriched milieu prevented the enhanced prothrombotic risk induced by FRD-elicited high PAI-1 production. PMID:23016136

    The purpose of this study was to examine sentinel lymph node (SLN) detection in patients with early stage cervical cancer using (99m)Tc phytate and patent blue dye and to compare our method with published findings utilizing other radioisotopic tracers. A total of 20 consecutive patients with cervical cancer scheduled for radical hysterectomy and total pelvic lymphadenectomy at our hospital underwent SLN detection study. The day before surgery, lymphoscintigraphy was performed with injection of 99m-technetium ((99m)Tc)-labeled phytate into the uterine cervix. At surgery, patients underwent lymphatic mapping with a gamma-detecting probe and patent blue injected into the same points as the phytate solution. At least one positive node was detected in 18 patients (90%). A total of 46 sentinel nodes were detected (mean, 2.3; range, 1-5). Most sentinel nodes were in one of the following sites: external iliac (21 nodes), obturator (15 nodes), and parametrial (7 nodes). Eleven (24%) sentinel nodes were detected only through radioactivity and two (4%) were detected only with blue dye. The sensitivity, specificity, and negative predictive value for SLN detection were all 100%. Nine published studies involving 295 patients had a summarized detection rate of 85%. Summarized sensitivity, specificity, and negative predictive value were 93%, 100%, and 99%, respectively. Combination of (99m)Tc phytate and patent blue is effective in SLN detection in early stage cervical cancer.

    The effect of a high fat diet in stimulating adipocyte proliferation, as measured by the incorporation of [3H]-thymidine into fat cell DNA, was studied in 22-month-old female Sprague-Dawley rats. Rats were fed a low fat (n = 10) or a high fat diet (n = 9) for a total of six days. On days 4 and 5 of dietary manipulation, rats were injected with 80 microCi/100 g body weight of [3H]-thymidine. Rats were continued on their respective diets for one more day, starved for 72 h and then refed a stock diet for three weeks in order to increase turnover of stroma cells, thus diluting the specific activity of stromal DNA with minimal effect on specific activity of fat cell DNA. The diet groups did not differ significantly with respect to body masses, food intake, parametrial (PARA) and retroperitoneal (RP) depot masses, cell number or cell size. The specific activity of DNA in both PARA and RP depots was greater in the adipocyte than in the stromavascular fraction. Specific activity of fat cells was significantly greater from rats fed the high fat than the low fat diet in both PARA and RP depots. Radioautography of adipose tissue confirmed that there was a greater percentage of adipocyte nuclei labeled in the rats fed the high fat diet. Also, there were few labeled nuclei found in stroma cells. In conclusion, older female rats increased adipocyte proliferation when fed a high fat diet.

  8. Highly differentiated keratinizing squamous cell cancer of the cervix: a rare, locally aggressive tumor not associated with human papillomavirus or squamous intraepithelial lesions.

    The purpose of this study is to report an unusual variant of cervical squamous cell carcinoma, not associated with either human papillomavirus infection or antecedent squamous intraepithelial lesions. Five women had a diagnosis of invasive cervical cancer discovered at hysterectomy performed for prolapse (two cases), leiomyoma (one case), or a vaginal fistula (two cases). The women ranged in age from 47 to 78 years (mean 59 years). Four of the five had a history of normal Papanicolaou (Pap) smears; the other had a Pap smear diagnosis of atypical squamous cells of undetermined significance (ASCUS). All had large cervical tumors (two with parametrial involvement and one with vaginal involvement) that showed extensive keratin formation, an inverted pattern of growth, and, except for one case, minimal cytologic atypia. There was extensive hyperkeratosis and parakeratosis adjacent to each tumor; none had evidence of squamous intraepithelial lesion. Human papillomavirus testing by polymerase chain reaction in situ hybridization and reverse-transcribed polymerase chain reaction in situ was negative in each case, compared with a detection rate of 107 of 108 (99%) for squamous intraepithelial lesion-associated cervical squamous cell and adenocarcinomas. Two of the women died of extensive local recurrence; two other women were recently diagnosed. We conclude that highly differentiated keratinizing squamous cell carcinoma of the cervix is a rare entity not associated with human papillomavirus infection or squamous intraepithelial lesion and thus difficult to detect on routine cervical cancer screening.

    To determine the usefulness of placental magnetic resonance imaging (MRI) in the diagnosis and surgical management of abnormal placentation. Retrospective follow-up. Buenos Aires, Argentina. 547 pregnant women. In all cases, a direct and reliable description of abnormal placentation features was obtained by the operating surgeon. Placental MRI was analyzed according to: (1) primary description, (2) invasion topography, (3) modification required to the surgical tactics or techniques and (4) by positive and negative predictive values. Ultrasound and MRI findings were compared with surgical results, which were considered a final diagnosis in relation to primary diagnostic indications. Placental MRI was obtained because of diagnostic doubt in 78 cases, for deep invasion diagnosis in 148 cases and to define the invasion area in 346 cases. Placental MRI allowed accurate demarcation and assessment of the degree of placental invasion, parametrial involvement and cervico-trigonal vascular hyperplasia, permitting changes in the surgical tactical approach. Ultrasound and MRI differences were associated with placenta previa, uterine scar thinning and use of different criteria for placental invasion through definitions or terminology. Six cases of false-negative and 11 of false-positive findings were reported. Placental MRI provides excellent characterization of the degree and extension of placental invasion. Its usefulness in cases of adherent placentation is directly associated to the therapeutic measures, especially where dissection maneuvers are needed.   10. Individualized Prediction of Overall Survival After Postoperative Radiation Therapy in Patients With Early-Stage Cervical Cancer: A Korean Radiation Oncology Group Study (KROG 13-03)

    Purpose: A nomogram is a predictive statistical model that generates the continuous probability of a clinical event such as death or recurrence. The aim of the study was to construct a nomogram to predict 5-year overall survival after postoperative radiation therapy for stage IB to IIA cervical cancer. Methods and Materials: The clinical data from 1702 patients with early-stage cervical cancer, treated at 10 participating hospitals from 1990 to 2011, were reviewed to develop a prediction nomogram based on the Cox proportional hazards model. Demographic, clinical, and pathologic variables were included and analyzed to formulate the nomogram. The discrimination andmore » calibration power of the model was measured using a concordance index (c-index) and calibration curve. Results: The median follow-up period for surviving patients was 75.6 months, and the 5-year overall survival probability was 87.1%. The final model was constructed using the following variables: age, number of positive pelvic lymph nodes, parametrial invasion, lymphovascular invasion, and the use of concurrent chemotherapy. The nomogram predicted the 5-year overall survival with a c-index of 0.69, which was superior to the predictive power of the International Federation of Gynecology and Obstetrics (FIGO) staging system (c-index of 0.54). Conclusions: A survival-predicting nomogram that offers an accurate level of prediction and discrimination was developed based on a large multi-center study. The model may be more useful than the FIGO staging system for counseling individual patients regarding prognosis.« less

  11. Cervical brachytherapy technique for locally advanced carcinoma of the cervix in a patient with septate uterus.

    To describe an approach to cervical brachytherapy in a patient with congenital septate uterus and locally advanced cervical carcinoma. The patient is a 34-year-old female with septate uterus presenting with pelvic pain. Workup demonstrated a stage IIB cervical adenocarcinoma with imaging evidence of an involved right external iliac lymph node. The patient received whole pelvic radiation, with concurrent weekly cisplatin (40 mg/m(2)), to a dose of 45 Gy in 25 fractions followed by a parametrial boost of 5.4 Gy and an additional nodal boost of 9 Gy. The patient was initiated on cervical brachytherapy following fraction 23 of pelvic radiation. To conform to her septated uterus, a Rotte-Y tandem was used. Additionally, 2 CT-compatible ovoids were placed in the vaginal apex to enhance dose distribution and coverage of the target volume. Each fraction of brachytherapy was performed with CT-based planning. A high-risk clinical target volume (HR-CTV) and normal structures were defined and constrained per American Brachytherapy Society (ABS) and Groupe Européen de Curiethérapie/European Society for Therapeutic Radiology and Oncology (GEC-ESTRO) guidelines. The brachytherapy dose was 27.5 Gy in 5 fractions of 5.5 Gy each, prescribed to the HR-CTV. Herein, we report the first documented case of cervical brachytherapy in a patient with septate uterus and locally advanced cervical carcinoma. Using CT-guided planning, in conjunction with the ABS and GEC-ESTRO guidelines, the patient was effectively treated with adapted cervical brachytherapy, meeting criteria for HR-CTV coverage and normal tissue tolerances.

  12. Application of carbon nanoparticles in laparoscopic sentinel lymph node detection in patients with early-stage cervical cancer.

    To investigate the value of carbon nanoparticles in identifying sentinel lymph nodes in early-stage cervical cancer. From January 2014 to January 2016, 40 patients with cervical cancer stage IA2-IIA, based on the International Federation of Gynecology and Obstetrics (FIGO) 2009 criteria, were included in this study. The normal cervix around the tumor was injected with a total of 1 mL of carbon nanoparticles (CNP)at 3 and 9 o'clock. All patients then underwent laparoscopic pelvic lymph node dissection and radical hysterectomy. The black-dyed sentinel lymph nodes were removed for routine pathological examination and immunohistochemical staining. Among the 40 patients, 38 patients had at least one sentinel lymph node (SLN). The detection rate was 95% (38/40). One hundred seventy-three SLNs were detected with an average of 3.9 SLNs per side. 25 positive lymph nodes, which included 21 positive SLNs, were detected in 8 (20%) patients. Sentinel lymph nodes were localized in the obturator (47.97%), internal lilac (13.87%), external lilac (26.59%), parametrial (1.16%), and common iliac (8.67%) regions. The sensitivity of the SLN detection was 100% (5/5), the accuracy was 97.37% (37/38), and the negative predictive value was 100. 0% and the false negative rate was 0%. Sentinel lymph nodes can be used to accurately predict the pathological state of pelvic lymph nodes in early cervical cancer. The detection rates and accuracy of sentinel lymph node were high. Carbon nanoparticles can be used to trace the sentinel lymph node in early cervical cancer.

    Most cervical cancer patients worldwide receive surgical treatments, and yet the current International Federation of Gynecology and Obstetrics (FIGO) staging system do not consider surgical-pathologic data. We propose a more comprehensive and prognostically valuable surgical-pathologic staging and scoring system (SPSs). Records from 4,220 eligible cervical cancer cases (Cohort 1) were screened for surgical-pathologic risk factors. We constructed a surgical-pathologic staging and SPSs, which was subsequently validated in a prospective study of 1,104 cervical cancer patients (Cohort 2). In Cohort 1, seven independent risk factors were associated with patient outcome: lymph node metastasis (LNM), parametrial involvement, histological type, grade, tumor size, stromal invasion, and lymph-vascular space invasion (LVSI). The FIGO staging system was revised and expanded into a surgical-pathologic staging system by including additional criteria of LNM, stromal invasion, and LVSI. LNM was subdivided into three categories based on number and location of metastases. Inclusion of all seven prognostic risk factors improves practical applicability. Patients were stratified into three SPSs risk categories: zero-, low-, and high-score with scores of 0, 1 to 3, and ≥4 (P=1.08E-45; P=6.15E-55). In Cohort 2, 5-year overall survival (OS) and disease-free survival (DFS) outcomes decreased with increased SPSs scores (P=9.04E-15; P=3.23E-16), validating the approach. Surgical-pathologic staging and SPSs show greater homogeneity and discriminatory utility than FIGO staging. Surgical-pathologic staging and SPSs improve characterization of tumor severity and disease invasion, which may more accurately predict outcome and guide postoperative therapy.

    Recent data have shown high rates of clinical and pathologic responses to neoadjuvant radiation therapy for locally advanced endometrial cancer. There are limited data on the surgical outcomes of these patients in the era of modern radiation and surgical techniques. We sought to characterize surgical outcomes after extrafascial hysterectomy in this population. Patients with endometrial cancer of all histologies clinically involving the cervix or parametria treated with neoadjuvant brachytherapy followed by extrafascial hysterectomy from 1999 to 2014 were identified. Patient charts were reviewed for data regarding treatment characteristics and postoperative outcomes. Pearson χ and logistic regression analyses were used to assess correlations between surgical complications and treatment-related variables. Twenty-nine patients met inclusion criteria. Mean operating time for the cohort was 115 minutes. Mean estimated blood loss was 100 mL. No visceral injuries occurred. Mean length of hospital stay was 1 and 4 days for the minimally invasive and laparotomy groups, respectively. Rates of postoperative ileus, blood transfusion, wound infection, and readmission were 3%, 3%, 6%, and 3%, respectively. No case of prolonged urodynamic dysfunction was noted. The rate of vaginal complications was significantly higher in the group of patients who underwent minimally invasive surgery as compared with laparotomy (33% vs 5%, P < 0.041). These data support adjuvant extrafascial hysterectomy after neoadjuvant radiotherapy for endometrial cancer with cervical or parametrial involvement as a safe and viable procedure, with low rates of postoperative complications. Extra care should be taken when closing the vaginal cuff to reduce the risk of vaginal cuff complications.

    In the management of early stage cervical cancer, knowledge about the prognosis is critical. Although many factors have an impact on survival, their relative importance remains controversial. This study aims to develop a prognostic model for survival in early stage cervical cancer patients and to reconsider grounds for adjuvant treatment. A multivariate Cox regression model was used to identify the prognostic weight of clinical and histological factors for disease-specific survival (DSS) in 710 consecutive patients who had surgery for early stage cervical cancer (FIGO [International Federation of Gynecology and Obstetrics] stage IA2-IIA). Prognostic scores were derived by converting the regression coefficients for each prognostic marker and used in a score chart. The discriminative capacity was expressed as the area under the curve (AUC) of the receiver operating characteristic. The 5-year DSS was 92%. Tumor diameter, histological type, lymph node metastasis, depth of stromal invasion, lymph vascular space invasion, and parametrial extension were independently associated with DSS and were included in a Cox regression model. This prognostic model, corrected for the 9% overfit shown by internal validation, showed a fair discriminative capacity (AUC, 0.73). The derived score chart predicting 5-year DSS showed a good discriminative capacity (AUC, 0.85). In patients with early stage cervical cancer, DSS can be predicted with a statistical model. Models, such as that presented here, should be used in clinical trials on the effects of adjuvant treatments in high-risk early cervical cancer patients, both to stratify and to include patients.     The goal of this study is to identify predictive factors in patients with a diagnosis of early-stage cervical cancer after simple hysterectomy in order to avoid a radical parametrectomy. A retrospective review was performed of all patients who underwent radical parametrectomy and bilateral pelvic lymphadenectomy at MD Anderson Cancer Center and at the Instituto de Cancerologia Las Americas in Medellin, Colombia from December 1999 to September 2017. We sought to determine the outcomes in patients diagnosed with low-risk factors (squamous, adenocarcinoma or adenosquamous lesions<2cm in size, and invading<10mm) undergoing radical parametrectomy and pelvic lymphadenectomy. A total of 30 patients were included in the study. The median age was 40.4years (range; 26-60) and median body mass index (BMI) was 26.4kg/m 2 (range; 17.7-40.0). A total 22 patients had tumors<1cm and 8 had tumors between 1 and 2cm. A total of 6 (33%) of 18 patients had evidence of lymph-vascular invasion (LVSI). No radical parametrectomy specimen had residual tumor, involvement of the parametrium, vaginal margin positivity, or lymph node metastasis. None of the patients received adjuvant therapy. After a median follow-up of 99months (range; 6-160) only one patient recurred. Radical parametrectomy may be avoided in patients with low-risk early-stage cervical cancer detected after a simple hysterectomy.   17. When Does Neoadjuvant Chemotherapy Really Avoid Radiotherapy? Clinical Predictors of Adjuvant Radiotherapy in Cervical Cancer.

    The aim of this study was to identify clinical variables that may predict the need for adjuvant radiotherapy after neoadjuvant chemotherapy (NACT) and radical surgery in locally advanced cervical cancer patients. A retrospective series of cervical cancer patients with International Federation of Gynecology and Obstetrics (FIGO) stages IB2-IIB treated with NACT followed by radical surgery was analyzed. Clinical predictors of persistence of intermediate- and/or high-risk factors at final pathological analysis were investigated. Statistical analysis was performed using univariate and multivariate analysis and using a model based on artificial intelligence known as artificial neuronal network (ANN) analysis. Overall, 101 patients were available for the analyses. Fifty-two (51 %) patients were considered at high risk secondary to parametrial, resection margin and/or lymph node involvement. When disease was confined to the cervix, four (4 %) patients were considered at intermediate risk. At univariate analysis, FIGO grade 3, stage IIB disease at diagnosis and the presence of enlarged nodes before NACT predicted the presence of intermediate- and/or high-risk factors at final pathological analysis. At multivariate analysis, only FIGO grade 3 and tumor diameter maintained statistical significance. The specificity of ANN models in evaluating predictive variables was slightly superior to conventional multivariable models. FIGO grade, stage, tumor diameter, and histology are associated with persistence of pathological intermediate- and/or high-risk factors after NACT and radical surgery. This information is useful in counseling patients at the time of treatment planning with regard to the probability of being subjected to pelvic radiotherapy after completion of the initially planned treatment.

  19. Breast cancer with synchronous massive metastasis in the uterine cervix: a case report and review of the literature.

    Metastatic breast cancer is rare in the female genital tract, and when present it more commonly tends to involve ovary or endometrium; uterine cervix is only occasionally involved. This condition poses differential diagnostic problems in the settings of clinical and pathological investigations. An asymptomatic 78-year-old woman came to our attention in the context of routine gynecological surveillance; clinical examination disclosed enlarged uterine body and cervix. Our patient then underwent computed tomography and magnetic resonance imaging that outlined the possibility of cervical cancer with parametrial involvement. Moreover, a suspect mass was found on the mammogram in the left breast. Breast surgical excision was performed, which revealed invasive breast carcinoma, while synchronous cervical biopsy discovered distant metastasis in the uterine cervix. On histological examination, both lesions showed non-cohesive architectural pattern consistent with lobular morphology; anyway, to rule out primary poorly differentiated cervical cancer, appropriate immunohistochemical panel was performed, which confirmed the mammary derivation of the tumor. Due to disseminate disease, the patient underwent multisystemic medical treatment including radiotherapy, chemotherapy and hormone therapy, and she is still alive at 30-month follow-up. Genital tract metastases in patients with known breast carcinoma can present with abnormal vaginal bleeding, but they often are asymptomatic. Therefore, only strict gynecological surveillance of these patients can permit early detection of these secondary lesions. Aggressive treatment of isolated cervical metastasis should be performed when feasible; otherwise, systemic chemotherapy with taxane could be sufficient in increasing survival. It should be emphasized that, in most cases, only accurate immunohistochemical investigation, particularly if performed on the primary lesion as well, can solve differential diagnostic problems and allow the

Results In Cohort 1, seven independent risk factors were associated with patient outcome: lymph node metastasis (LNM), parametrial involvement, histological type, grade, tumor size, stromal invasion, and lymph-vascular space invasion (LVSI). The FIGO staging system was revised and expanded into a surgical-pathologic staging system by including additional criteria of LNM, stromal invasion, and LVSI. LNM was subdivided into three categories based on number and location of metastases. Inclusion of all seven prognostic risk factors improves practical applicability. Patients were stratified into three SPSs risk categories: zero-, low-, and high-score with scores of 0, 1 to 3, and ≥4 (P=1.08E-45; P=6.15E-55). In Cohort 2, 5-year overall survival (OS) and disease-free survival (DFS) outcomes decreased with increased SPSs scores (P=9.04E-15; P=3.23E-16), validating the approach. Surgical-pathologic staging and SPSs show greater homogeneity and discriminatory utility than FIGO staging. Conclusions Surgical-pathologic staging and SPSs improve characterization of tumor severity and disease invasion, which may more accurately predict outcome and guide postoperative therapy.   1. [Deep infiltrating endometriosis surgical management and pelvic nerves injury].

    Deep pelvic endometriosis surgery may need substantial excisions, which in turn expose to risks of injury to the pelvic nerves. To limit functional complications, nerve-sparing surgical techniques have been developed but should be adapted to the specific multifocal character of endometriotic lesions. The objective was to identify the anatomical areas where the pelvic nerves are most at risk of injury during endometriotic excisions. The Medline and Embase databases have been searched for available literature using the keywords "hypogastric nerve or hypogastric plexus [Mesh] or autonomic pathway [Mesh], anatomy, endometriosis, surgery [Mesh]". All relevant French and English publications, selected based on their available abstracts, have been reviewed. Five female adult fresh cadavers have been dissected to localize the key anatomical areas where the pelvic nerves are most at risk of injury. Six anatomical areas of high risk for pelvic nerves have been identified, analysed and described. Pelvic nerves can be damaged during the dissection of retrorectal space and the anterolateral rectal excision. Furthermore, before an uterosacral ligament excision, a parametrial excision, a colpectomy or a dissection of the vesico-uterine ligament, the hypogastric nerves, splanchnic nerves, inferior hypogastric plexus and its efferent pathways must be mapped out to avoid injury. The distance between the deep uterin vein and the pelvic splanchnic nerves were measured on four cadavers and varied from 2.5cm to 4cm. Six key anatomical pitfalls must be known in order to limit the functional complications of the endometriotic surgical excision. Applying nerve-sparing surgical techniques for endometriosis would lead to less urinary functional complications and a better short-term postoperative satisfaction.     Introduction: For patients with cervical cancer, intensity-modulated radiation therapy (IMRT) improves target coverage and allows dose escalation while reducing the radiation dose to organs at risk (OARs). In this study, we compared dosimetric parameters among 3-dimensional conformal radiotherapy (3D-CRT), “step-and-shoot” IMRT, and volumetric intensity-modulated arc radiotherapy (VMAT) in a series of patients with cervical cancer receiving definitive radiotherapy. Computed tomography (CT) scans of 10 patients with histologically proven cervical cancer treated with definitive radiation therapy (RT) from December 2008 to March 2010 at our department were selected for this study. The gross tumor volume (GTV) and clinical target volume (CTV)more » were delineated following the guidelines of the Gyn IMRT consortium that included cervix, uterus, parametrial tissues, and the pelvic nodes including presacral. The median age was 57 years (range: 30 to 85 years). All 10 patients had squamous cell carcinoma with Federation of Gynecology and Obstetrics (FIGO) stage IB-IIIB. All patients were treated by VMAT. OAR doses were significantly reduced for plans with intensity-modulated technique compared with 3D-CRT except for the dose to the vagina. Between the 2 intensity-modulated techniques, significant difference was observed for the mean dose to the small intestine, to the benefit of VMAT (p < 0.001). There was no improvement in terms of OARs sparing for VMAT although there was a tendency for a slightly decreased average dose to the rectum: − 0.65 Gy but not significant (p = 0.07). The intensity modulation techniques have many advantages in terms of quality indexes, and particularly OAR sparing, compared with 3D-CRT. Following the ongoing technologic developments in modern radiotherapy, it is essential to evaluate the intensity-modulated techniques on prospective studies of a larger scale.« less

    At the turn of the 19th and 20th century, on the areas of today northern and western Poland, the railway infrastructure was developing rapidly. In the late 80's of the previous century many of the railway lines, together with the whole infrastructure including bridges, were being closed down. The example of such practices can be the brick bridge located near Nojewo, Voivodeship wielkopolskie. This article presents the technical parameters of these bridges and their damages as well as ideas for the future development of the railway facilities. Na przełomie XIX i XX wieku na terenie dzisiejszej Polski zachodniej i północnej bardzo intensywnie rozwijana była infrastruktura kolejowa. W efekcie tych działań powstała sieć dróg żelaznych o dużym zagęszczeniu. Pod koniec lat 80-tych ubiegłego stulecia wiele z tych linii było stopniowo zamykanych. Na skutek tych przemian wiele linii oraz obiektów z nimi związanych, w tym obiekty mostowe, zostały pozostawione niekorzystnemu działaniu czasu. Przykładem takich wyłączonych z eksploatacji obiektów są mosty i wiadukty położone w okolicy miejscowości Nojewo. Wspomniane obiekty inżynierskie to ceglane konstrukcje sklepione znajdujące się w ciągu linii kolejowej nr 368 łączącej Szamotuły i Międzychód. W artykule zaprezentowano problem rewitalizacji nieużytkowanych mostów kolejowych na przykładzie obiektów w miejscowości Nojewo w województwie wielkopolskim. Opisano podstawowe parametry techniczne mostów oraz ich uszkodzenia. Przedstawiono propozycje zagospodarowania obiektów.

  4. Multimodal endocavitary ultrasound versus MRI and clinical findings in pre- and post-treatment advanced cervical cancer. Preliminary report.

    The aim of this study was to evaluate the use of pre and post-therapy transrectal and transvaginal ultrasonography (TRUS, TVUS) with contrast enhancement and strain elastography compared with clinical examination and magnetic resonance imaging (MRI) in the assessment of advanced stage cervical cancer. This was a prospective study, carried out over a period of nine months on subjects with advanced-stage cervical cancer (stage >/= IIB). All included patients were examined clinically and underwent abdomino-pelvic contrast enhanced MRI and multimodal US examinations (TRUS with strain elastography and contrast enhanced TVUS) at the time of diagnosis and after radiochemotherapy. Tumor size and staging at TRUS and TVUS was compared with the same data obtained by clinical examination and MRI. Pathology was the golden standard. Eight patients accomplished the inclusion criteria. In five cases the tumor stage was identical on clinical and MRI examinations. In all cases parametrial infiltration was diagnosed by all pre-treatment examinations. No significant differences were observed in tumor size between clinical, US and MRI exams either at baseline or post-therapy, in native or post-contrast examinations. The size of the tumor evaluated pre-treatment proved to be significantly smaller post-contrast in both US and MRI examinations compared with the native images. Post-therapy, no significant differences were observed on US measured tumor dimensions when comparing native with post-contrast images. Oppositely, significant smaller dimensions were observed on post-contrast MRI compared with native scans. TRUS is accurate in the estimation of pre-therapy cervical cancer dimension. The post therapy tumor evaluation is better performed with MRI. The use of intravenous contrast agents on both examinations did not improved the accuracy of tumor evaluation pre or post-therapy.

  5. A Prospective Cohort Study to Compare Treatment Results Between 2 Fractionation Schedules of High-Dose-Rate Intracavitary Brachytherapy (HDR-ICBT) in Patients With Cervical Cancer

    Purpose: To compare the treatment results of 2 fractionation schedules for high-dose-rate intracavitary brachytherapy (HDR-ICBT) in patients with cervical cancer. Methods and Materials: From June 2001 through January 2008, 267 patients with stage IB-IVA cervical cancer were enrolled in the study. All patients underwent 4-field pelvic irradiation and HDR-ICBT. The median central and parametrial doses were 39.6 Gy and 45 Gy, respectively. Patient underwent either 6 Gy Multiplication-Sign 4 (HDR-4) (n=144) or 4.5 Gy Multiplication-Sign 6 (HDR-6) (n=123) to point A of ICBT using {sup 192}Ir isotope twice weekly. The rates of overall survival, locoregional failure, distant metastasis, proctitis, cystitis,more » and enterocolitis were compared between HDR-4 and HDR-6. Results: There were no significant differences in the demographic data between HDR-4 and HDR-6 except for total treatment time. The 5-year proctitis rates were 23.0% and 21.5% in HDR-4 and HDR-6 (P=.399), respectively. The corresponding rates of grade 2-4 proctitis were 18.7% and 9.6% (P=.060). The corresponding rates of grades 3-4 proctitis were 5.2% and 1.3% (P=.231). Subgroup analysis revealed that HDR-4 significantly increased grade 2-4 proctitis in patients aged {>=}62 years old (P=.012) but not in patients aged <62 years (P=.976). The rates of overall survival, locoregional failure, distant metastasis, cystitis, and enterocolitis were not significantly different between HDR-4 and HDR-6 schedules. Conclusion: The small fraction size of HDR-ICBT is associated with grade 2 proctitis without compromise of prognosis in elderly patients. This schedule is suggested for patients who tolerate an additional 2 applications of HDR-ICBT.« less

  6. [Sentinel node detection in early stage of cervical carcinoma using 99mTc-nanocolloid and blue dye].

    The aim of the study was to analyse the feasibility of intraoperative sentinel lymph nodes (SLN) detection using gamma detection probe and blue dye in patients undergoing radical hysterectomy for treatment of early stage of cervical cancer. Prospective case observational study. In the period from May 2004 to February 2006 77 patients with early stage of cervical cancer who underwent a radical surgery were included into the study. Patients were divided into three groups according to the tumour volume. First group consists of patients FIGO IA2 and FIGO IB1 with tumour diameter less than 2 cm, second group tumours FIGO IB1 with tumour diameter more than 2 cm and third group stadium IB2. SLN was detected by blue dye and Tc99. Preoperative lymphoscintigraphy was done after Tc99 colloid injection, intraoperative detection was performed by visual observation and by hand-held gamma-detection probe. SLN were histologically and immunohistochemically analysed. A total number of 2764 lymph nodes with an average 36 and 202 SLN with an average 2.6 were identified. The SLN detection rate was 94.8% per patient and 85.1% for the side of dissection and depends on the tumor volume. SLN were identified in obturator area in 48%, in external iliac area in 15%, in common iliac and internal iliac both in 9%, in interiliac region in 8%, in praesacral region in 6% and in parametrial area in 5%. Metastatic disease was detected in 31 patients (40.2%), metastatic involvement of SLN only in 12 patients (15.6%). False negative rate was 2.6%, sensitivity and negative predictive value calculated by patient were 923% and 95.7%. Intraoperative lymphatic mapping using combination of technecium-99-labeled nanocolloid and blue dye are feasible, safe and accurate techniques to identified SLN in early stage of cervical cancer.

    Classification techniques have been widely used in different remote sensing applications and correct classification of mixed pixels is a tedious task. Traditional approaches adopt various statistical parameters, however does not facilitate effective visualisation. Data mining tools are proving very helpful in the classification process. We propose a visual mining based frame work for accuracy assessment of classification techniques using open source tools such as WEKA and PREFUSE. These tools in integration can provide an efficient approach for getting information about improvements in the classification accuracy and helps in refining training data set. We have illustrated framework for investigating the effects of various resampling methods on classification accuracy and found that bilinear (BL) is best suited for preserving radiometric characteristics. We have also investigated the optimal number of folds required for effective analysis of LISS-IV images. Techniki klasyfikacji są szeroko wykorzystywane w różnych aplikacjach teledetekcyjnych, w których poprawna klasyfikacja pikseli stanowi poważne wyzwanie. Podejście tradycyjne wykorzystujące różnego rodzaju parametry statystyczne nie zapewnia efektywnej wizualizacji. Wielce obiecujące wydaje się zastosowanie do klasyfikacji narzędzi do eksploracji danych. W artykule zaproponowano podejście bazujące na wizualnej analizie eksploracyjnej, wykorzystujące takie narzędzia typu open source jak WEKA i PREFUSE. Wymienione narzędzia ułatwiają korektę pół treningowych i efektywnie wspomagają poprawę dokładności klasyfikacji. Działanie metody sprawdzono wykorzystując wpływ różnych metod resampling na zachowanie dokładności radiometrycznej i uzyskując najlepsze wyniki dla metody bilinearnej (BL).

  8. Determination of rock-sample anisotropy from P- and S-wave traveltime inversion

    We determine anisotropy of a rock sample from laboratory measurements of P- and S-wave traveltimes using weak-anisotropy approximation and parametri-zation of the medium by a special set of anisotropy parameters. For the traveltime inversion we use first-order velocity expressions in the weak-anisotropy approximation, which allow to deal with P and S waves separately. Each wave is described by 15 anisotropy parameters, 9 of which are common for both waves. The parameters allow an approximate construction of separate P- or common S-wave phase-velocity surfaces. Common S wave concept is used to simplify the treatment of S waves. In order to obtain all 21 anisotropy parameters, P- and S-wave traveltimes must be inverted jointly. The proposed inversion scheme has several advantages. As a consequence of the use of weak-anisotropy approximation and assumed homogeneity of the rock sample, equations used for the inversion are linear. Thus the inversion procedure is non-iterative. In the approximation used, phase and ray velocities are equal in their magnitude and direction. Thus analysis whether the measured velocity is the ray or phase velocity is unnecessary. Another advantage of the proposed inversion scheme is that, thanks to the use of the common S-wave concept, it does not require identification of S-wave modes. It is sufficient to know the two S-wave traveltimes without specification, to which S-wave mode they belong. The inversion procedure is tested first on synthetic traveltimes and then used for the inversion of traveltimes measured in laboratory. In both cases, we perform first the inversion of P-wave traveltimes alone and then joint inversion of P- and S-wave traveltimes, and compare the results.

  9. Impact of Blood Transfusions on Survival of Locally Advanced Cervical Cancer Patients Undergoing Neoadjuvant Chemotherapy Plus Radical Surgery.

    Transfusions represent one of the main progresses of modern medicine. However, accumulating evidence supports that transfusions correlate with worse survival outcomes in patients affected by solid cancers. In the present study, we aimed to investigate the effects of perioperative blood transfusion in locally advanced cervical cancer. Data of consecutive patients affected by locally advanced cervical cancer scheduled to undergo neoadjuvant chemotherapy plus radical surgery were retrospectively searched to test the impact of perioperative transfusions on survival outcomes. Five-year survival outcomes were evaluated using Kaplan-Meier and Cox models. The study included 275 patients. Overall, 170 (62%) patients had blood transfusion. Via univariate analysis, we observed that transfusion correlated with an increased risk of developing recurrence (hazard ratio [HR], 2.2; 95% confidence interval [CI], 1.09-4.40; P = 0.02). Other factors associated with 5-year disease-free survival were noncomplete clinical response after neoadjuvant chemotherapy (HR, 2.99; 95% CI, 0.92-9.63; P = 0.06) and pathological (P = 0.03) response at neoadjuvant chemotherapy as well as parametrial (P = 0.004), vaginal (P < 0.001), and lymph node (P = 0.002) involvements. However, via multivariate analysis, only vaginal (HR, 3.07; 95% CI, 1.20-7.85; P = 0.01) and lymph node involvements (HR, 2.4; 95% CI, 1.00-6.06; P = 0.05) correlate with worse disease-free survival. No association with worse outcomes was observed for patients undergoing blood transfusion (HR, 2.71; 95% CI, 0.91-8.03; P = 0.07). Looking at factors influencing overall survival, we observed that lymph node status (P = 0.01) and vaginal involvement (P = 0.06) were independently associated with survival. The role of blood transfusions in increasing the risk of developing recurrence in LAAC patients treated by neoadjuvant chemotherapy plus radical surgery remains unclear; further prospective studies are warranted.

  10. Dosimetric Comparison of Bone Marrow-Sparing Intensity-Modulated Radiotherapy Versus Conventional Techniques for Treatment of Cervical Cancer

    Purpose: To compare bone marrow-sparing intensity-modulated pelvic radiotherapy (BMS-IMRT) with conventional (four-field box and anteroposterior-posteroanterior [AP-PA]) techniques in the treatment of cervical cancer. Methods and Materials: The data from 7 cervical cancer patients treated with concurrent chemotherapy and IMRT without BMS were analyzed and compared with data using four-field box and AP-PA techniques. All plans were normalized to cover the planning target volume with the 99% isodose line. The clinical target volume consisted of the pelvic and presacral lymph nodes, uterus and cervix, upper vagina, and parametrial tissue. Normal tissues included bowel, bladder, and pelvic bone marrow (PBM), which comprisedmore » the lumbosacral spine and ilium and the ischium, pubis, and proximal femora (lower pelvis bone marrow). Dose-volume histograms for the planning target volume and normal tissues were compared for BMS-IMRT vs. four-field box and AP-PA plans. Results: BMS-IMRT was superior to the four-field box technique in reducing the dose to the PBM, small bowel, rectum, and bladder. Compared with AP-PA plans, BMS-IMRT reduced the PBM volume receiving a dose >16.4 Gy. BMS-IMRT reduced the volume of ilium, lower pelvis bone marrow, and bowel receiving a dose >27.7, >18.7, and >21.1 Gy, respectively, but increased dose below these thresholds compared with the AP-PA plans. BMS-IMRT reduced the volume of lumbosacral spine bone marrow, rectum, small bowel, and bladder at all dose levels in all 7 patients. Conclusion: BMS-IMRT reduced irradiation of PBM compared with the four-field box technique. Compared with the AP-PA technique, BMS-IMRT reduced lumbosacral spine bone marrow irradiation and reduced the volume of PBM irradiated to high doses. Therefore BMS-IMRT might reduce acute hematologic toxicity compared with conventional techniques.« less

  11. Physical training improves visceral adipose tissue health by remodelling extracellular matrix in rats with estrogen absence: a gene expression analysis.

    Adipose tissue development is associated with modifications involving extracellular matrix remodelling, and metalloproteinases play a significant role in this process. Reduced circulating sexual hormones cause impacts on the size, morphology and functions of the adipose tissue, increasing susceptibility to diseases. This study investigated whether exercise training may be an alternative strategy to combat the effects promoted by estrogen decay through modulation in gene expression patterns in the extracellular matrix (ECM) of visceral adipose tissue of ovariectomized rats. Nulliparous rats (n = 40) were randomly distributed into four groups (n = 10/group): sham sedentary (Sh-S), sham resistance training (Sh-Rt), ovariectomized sedentary (Ovx-S) and ovariectomized resistance training (Ovx-Rt). The Sh-S animals did not have any type of training. The body mass and food intake, ECM gene expression, gelatinase MMP-2 activity and adipocyte area were measured. A lack of estrogen promoted an increase in body mass, food intake and the visceral, parametrial and subcutaneous adipocyte areas. The ovariectomy upregulated the expression of MMP-2, MMP-9, TGF-β, CTGF, VEGF-A and MMP-2 activity. On the other hand, resistance training decreased the body mass, food intake and the adipocyte area of the three fat depots analysed; upregulated TIMP-1, VEGF-A and MMP-2 gene expression; downregulated MMP-9, TGF-β and CTGF gene expression; and decreased the MMP-2 activity. We speculate that resistance training on a vertical ladder could play an important role in maintaining and remodelling ECM by modulation in the ECM gene expression and MMP-2 activity, avoiding its destabilization which is impaired by the lack of estrogen. © 2017 The Authors.     This review describes the possibility of development a new lignite deposit in northern Kosovo lignite basin - Sibovc. Analysis of the initial state briefly evaluates Kosovo energy sector, geomorphological conditions and quality of lignite from Sibovc deposit. With using Dataminesoft it was created geological model and approximate calculation of lignite reserves in the deposit. The data obtained from Dataminesoft were used as starting points of the financial analysis of project. The result of the analysis is exactly describe the qualitative and quantitative characteristics of deposit Sibovc compared to other deposits in the area and creating of geological model with productive horizons deposit of lignite. Based on these data lignite deposit Sibovc was classified, according to the classification of deposits the UN, as economical. W pracy tej omówiono możliwości udostępnienia nowego obszaru wybierania złoża węgla brunatnego (lignitu) w północnej części zagłębia węgla brunatnego Sibovc w Kosowie. W analizie stanu początkowego krótko scharakteryzowano sektor energetyczny Kosowa, warunki geo-morfologiczne oraz parametry jakościowe węgla brunatnego z zagłębia Sibovc. Przy pomocy pakietu Dataminesoft stworzono model geologiczny i przeprowadzono przybliżone obliczenia zasobów węgla brunatnego w złożu. Dane uzyskane przy zastosowaniu pakietu Dataminesoft zostały następnie wykorzystane jako dane wejściowe do analizy finansowej przedsięwzięcia. Na podstawie wyników analizy uzyskuje się jakościową i ilościową charakterystykę złoża w odniesieniu do pozostałych złóż w regionie. Opracowano model geologiczny ze szczegółowym wskazaniem poziomów wybierania lignitu. W oparciu o te dane dokonano klasyfikacji złoża węgla brunatnego (lignitu) w Sibovc zgodnie z międzynarodowymi zasadami klasyfikacji wykazując, że złoże będzie ekonomiczne.

Of 2612 patients who received pelvic irradiation for carcinoma of the uterine cervix, 40 developed fractures of the femoral neck, an incidence of 1.5%. Sixteen of the 40 patients had bilateral fractures. The average age of the patients was 58.5 yr at the time of irradiation. The average irradiation dose was approximates 3600 r (parametrial dose), the largest dose being 4235 r. The average interval from irradiation to the onset of pain in the hip was 36.6 months (3 to 240 months) and from the onsetmore » of pain to diagnosis, 3 months. Forty-six surgical procedures were performed: 36 as primary treatment and 9 because of failure or complications of the first procedure. The average follow-up time of these patients was 6.9 yr. The presenting complaint was usually spontaneous onset of pain in the groin and medial portion of the thigh. Initially the physical examination often revealed only restriction of internal rotation of the affected hip, and the roentgenograms appeared normal in some instances. Subsequently, a change in bone density was noted at the inferior aspect of the femoral neck, denoting an adduction type of fracture. The displaced fractures resembled the traumatic adduction fractures of the femoral neck seen in patients who had not received irradiation. Acetabular changes were also noted, characterized by marked osteoporosis and occasionally fracture of the acetabulum. Seven methods of primary therapy were used to treat these patients: no treatment, nonsurgical measures (crutches or bedrest), internal fixation, bone- grafting (with and without additional fixation), osteotomy, arthroplasty, and the insertion of a prosthesis.     2,4,5,2',4',5'-Hexachlorobiphenyl (6-CB) is mobilized from rodent tissues during the lipid depletion associated with food restriction or lactation, the latter condition resulting in the substantial elimination of the maternal body burden of the chemical to nursing offspring. The present study was undertaken to determine whether the rate and/or magnitude of accumulation of 6-CB in nursing offspring differed with time following PCB administration to the maternal animal. Female ICR mice were administered two doses of 6-CB. Group I animals received (14C)-6-CB as weanlings (15-20 g) followed by unlabeled 6-CB 5 weeks later, after mating, on Day 1 of gestation. Group II receivedmore » unlabeled 6-CB as weanlings and (14C)-6-CB on Day 1 of gestation. Thus, 14C identified the mobilization and elimination of either the first or the second dose of 6-CB in the treatment groups (I = (14C)-6-CB, 6-CB; II = 6-CB, (14C)-6-CB). Both groups of animals retained approximately 80% of the administered radiolabeled dose. The tissue distribution of (14C)-6-CB in group II as a percentage of the body burden was not different from that in group I as determined from maternal tissue concentrations on Day 14 of gestation. The percentage of the maternal body burden of (14C)-6-CB accumulated in suckling offspring of group II mothers was significantly greater than that in group I offspring on Day 1 (I, 2.2 +/- 0.5%; II, 3.5 +/- 0.4%), Day 3 (I, 14.8 +/- 1.9%; II, 24.6 +/- 2.7%), Day 5 (I, 16.8 +/- 1.4%; II, 24.8 +/- 0.8%), and Day 12 (I, 32.3 +/- 0.5%; II, 45.5 +/- 1.7%) postpartum.     Lymph node metastasis is a significant predictive factor for disease recurrence and survival in cervical cancer patients. Given the importance of lymph node metastasis, it is imperative that patients harboring metastasis are identified and can undergo appropriate treatment. Sentinel lymph node (SLN) mapping has drawn attention as a lymph node mapping technique. We evaluated the feasibility and efficacy of (SLN) mapping using indocyanine green (ICG) in cervical cancer. We performed a single-center, retrospective study of 103 surgically treated cervical cancer patients who underwent SLN mapping. After using ICG to detect SLN during surgery, we removed the SLNs followed by laparoscopic or robotic-assisted radical surgery and bilateral pelvic lymphadenectomy. Stage IB1 was the most common (61.17%). At least one SLN was detected in all cases. Eighty-eight patients (85.44%) had bilateral pelvic SLNs. The mean number of SLN per patient was 2.34. The side-specific sensitivity was 71.43%, the specificity was 100%, the negative predictive value (NPV) was 93.98%, and the false negative rate (FNR) was 28.57%. In cases of tumors smaller than 2 cm with negative lymph node metastasis on imaging, the study revealed a side-specific sensitivity of 100%, a specificity of 100%, a NPV of 100%, and a FNR of 0%. Large tumor size (≥ 4 cm), a previous history of a loop electrosurgical excision procedure (LEEP), depth of invasion (≥ 50%), the microscopic parametrial (PM) invasion, and vaginal extension were significantly associated with the false-negative detection of SLN. Moreover, the microscopic PM invasion was the only risk factor of the false-negative detection of SLN in multivariate analysis. SLN mapping with ICG in cervical cancer is feasible and has high detection rate. The sensitivity of 100% was high enough to perform SLN biopsy alone in an early stage in which the tumor is less than 2 cm, with no lymphadenopathy on image examination.     Purpose: To evaluate volumetric coverage of a Mick Radionuclear titanium Split-Ring applicator (SRA) with/without interstitial needle compared to an intracavitary Vienna applicator (VA), interstitial-intracavitary VA, and intracavitary ring and tandem applicator (RTA). Methods: A 57 year-old female with FIGO stage IIB cervical carcinoma was treated following chemoradiotherapy (45Gy pelvic and 5.4Gy parametrial boost) with highdose- rate (HDR) brachytherapy to 30Gy in 5 fractions using a SRA. A single interstitial needle was placed using the Ellis Interstitial Cap for the final three fractions to increase coverage of left-sided gross residual disease identified on 3T-MRI. High-risk (HR) clinical target volume (CTV) andmore » intermediate-risk (IR) CTV were defined using axial T2-weighted 2D and 3D MRI sequences (Philips PET/MRI unit). Organs-at-risks (OARs) were delineated on CT. Oncentra planning system was used for treatment optimization satisfying GEC-ESTRO guidelines for target coverage and OAR constraints. Retrospectively, treatment plans (additional 20 plans) were simulated using intracavitary SRA (without needle), intracavitary VA (without needle), interstitial-intracavitary VA, and intracavitary RTA with this same patient case. Plans were optimized for each fraction to maintain coverage to HR-CTV. Results: Interstitial-intracavitary SRA achieved the following combined coverage for external radiation and brachytherapy (EQD2): D90 HR-CTV =94.6Gy; Bladder-2cc =88.9Gy; Rectum-2cc =65.1Gy; Sigmoid-2cc =48.9Gy; Left vaginal wall (VW) =103Gy, Right VW =99.2Gy. Interstitial-intracavitary VA was able to achieve identical D90 HR-CTV =94.6Gy, yet Bladder-2cc =91.9Gy (exceeding GEC-ESTRO recommendations of 2cc<90Gy) and Left VW =120.8Gy and Right VW =115.5Gy. Neither the SRA nor VA without interstitial needle could cover HR-CTV adequately without exceeding dose to Bladder-2cc.     The aim of this study was to investigate the feasibility of sentinel lymph node (SLN) identification using radioisotopic lymphatic mapping with technetium-99 m-labeled phytate in patients undergoing radical hysterectomy with pelvic lymphadenectomy for treatment of early cervical cancer. Between July 2001 and February 2003, 56 patients with cervical cancer FIGO stage I (n = 53) or stage II (n = 3) underwent sentinel lymph node detection with preoperative lymphoscintigraphy ((99m)Tc-labeled phytate injected into the uterine cervix, at 3, 6, 9, and 12 o'clock, at a dose of 55-74 MBq in a volume of 0.8 ml) and intraoperative lymphatic mapping with a handheld gamma probe. Radical hysterectomy was aborted in three cases because parametrial invasion was found intraoperatively and we performed only sentinel node resection. The remaining 53 patients underwent radical hysterectomy with complete pelvic lymphadenectomy. Sentinel nodes were detected using a handheld gamma-probe and removed for pathological assessment during the abdominal radical hysterectomy and pelvic lymphadenectomy. One or more sentinel nodes were detected in 52 out of 56 eligible patients (92.8%). A total of 120 SLNs were detected by lymphoscintigraphy (mean 2.27 nodes per patient) and intraoperatively by gamma probe. Forty-four percent of SLNs were found in the external iliac area, 39% in the obturator region, 8.3% in interiliac region, and 6.7% in the common iliac area. Unilateral sentinel nodes were found in thirty-one patients (59%). The remaining 21 patients (41%) had bilateral sentinel nodes. Microscopic nodal metastases were confirmed in 17 (32%) cases. In 10 of these patients, only SLNs had metastases. The 98 sentinel nodes that were negative on hematoxylin and eosin were submitted to cytokeratin immunohistochemical analysis.     The oncogenic role of the fibroblast growth factor receptor (FGFR) has been recognized in a number of different cancer types. However, the prognostic significance of FGFRs has not been elucidated yet in cervical cancer. In the present study, we investigate the expression of FGFRs and their prognostic value in cervical cancer patients. FGFR1, FGFR2, FGFR3, and FGFR4 expression was determined by immunohistochemistry in conjunction with quantitative digital image analysis of 336 formalin-fixed, paraffin-embedded cervical cancer tissues and 61 normal cervical tissues, as well as NCI60 cell microarray. Subsequently, the association between clinicopathological characteristics and patient survival was assessed. FGFRs proteins were differentially expressed in the NCI60 cell line panel and showed considerable correlation between protein and mRNA expression. The expression of FGFR1, FGFR2, and FGFR4 were higher in cancer tissues than in normal tissues, whereas the expression of FGFR3 was higher in normal tissues. FGFR1 was highly expressed in adeno-/adenosquamous carcinoma (P = 0.020), while FGFR2, FGFR3, and FGFR4 expression were more prominent in squamous cell carcinoma (P < 0.001, P < 0.001, and P = 0.020, respectively). FGFR2 expression was significantly higher in small sized tumors (P = 0.020). Additionally, high FGFR2 and FGFR4 were correlated with negative lymph node metastasis (P = 0.048 and P = 0.040, respectively). FGFR1, FGFR2, and FGFR3 were highly expressed in tumors without parametrial involvement (P = 0.030, P = 0.005, and P = 0.010, respectively). In survival analysis, high expressions of FGFR2, FGFR3, and FGFR4 was associated with longer disease-free survival (P = 0.006, P = 0.035, P = 0.001, respectively) and overall survival (P = 0.003, P = 0.002, P = 0.003, respectively). Podczas pracy silników do atmosfery emitowane są między innymi następujące substancje: tlenki azotu (NOx), ditlenek siarki (SO2), tlenek węgla (CO), pył PM10, amoniak, benzo(a)piren (B(a)P), benzen, toluen, ksylen, formaldehyd, acetaldehyd, akroleina. Skutkiem emisji mogą być przekroczenia dopuszczalnych, określonych prawem wartości stężeń substancji w powietrzu. W artykule skoncentrowano się na ocenie wpływu na jakość powietrza atmosferycznego emisji substancji pyłowo-gazowych towarzyszących procesowi szczelinowania skał łupkowych. Badania miały charakter modelowy. W oparciu o analizę przebiegu szeregu procesów szczelinowania hydraulicznego wykonywanych na wiertniach zlokalizowanych w Polsce i za granicą zbudowano przykładowy model procesu, który posłużył do wykonania symulacji propagacji zanieczyszczeń emitowanych substancji w powietrzu w zadeklarowanym obszarze obliczeniowym stanowiącym teren o wymiarach 1150 m × 1150 m. Dla urealnienia wyników obliczeń umiejscowiono model badanego obszaru w okolicach Gdańska i przypisano mu charakterystyczne dla tego rejonu parametry meteorologiczne i orograficzne. W środku analizowanego obszaru zlokalizowano teren wiertni o wymiarach 150 m x 150 m. Pole emisji kształtowane było przez 12 silników wysokoprężnych napędzających agregaty pompowe, każdy o mocy 1680 kW. Czas pracy pojedynczego silnika ustalono na 52 godziny w roku (13 zabiegów szczelinowania, każdy trwający 4 godziny). W obliczeniach założono, że podczas trwania szczelinowania wszystkie silniki będą pracowały równocześnie i będą obciążone w 100 %.   1. Laparoscopic Sentinel Lymph Node Mapping with Indocyanine Green Using the iSpies Platform: Initial Experience Argentina.

    To demonstrate the initial experience in Argentina using the iSpies indocyanine green (ICG) platform in sentinel lymph node mapping in patients with early-stage cervical cancer. Step-by-step demonstration of the technique using a video and pictures (educative video) (Canadian Task Force classification III). Laparoscopic and robotic sentinel lymph node mapping using ICG has been shown to be safe and feasible; however, in developing countries, the opportunities to use fluorescent imaging through a minimally invasive approach are very limited, given the cost restrictions of acquiring the near-infrared technology and the fluorescent dyes. A 47-year-old woman presented with a stage IB1 squamous cervical cancer. Physical examination revealed a 1.5-cm tumor without evidence of parametrial involvement. Magnetic resonance imaging did not show any evidence of metastatic disease. The patient underwent laparoscopic radical hysterectomy with sentinel lymph node mapping. On laparoscopic exposure of the pelvic spaces, a cervical injection of ICG (1 mL superficial and deep) was administered using a spinal needle at the 3 o'clock and 9 o'clock positions. Sentinel lymph node mapping was then performed using the ICG (Pulsion Medical Systems, Feldkirchen, Germany) and an iSpies near-infrared camera (Karl Storz Endoskope, Tuttlingen, Germany). Bilateral sentinel lymph nodes were detected on the left external iliac artery and in the right obturator space. Both were confirmed ex vivo. The total operative time was 170 minutes. No intraoperative or postoperative complications were reported, and the patient was discharged at 48 hours after surgery. Estimated blood loss was minimal. Sentinel lymph node mapping alone is not the standard of care in our institution, and thus bilateral lymphadenectomy was performed.     One of the major advantages of light gauge steel structures made of cold-formed steel sections is their low weight so the production of typical single-storey steel structures of this kind of profiles is still rising. The well known profiles, e.o. Z-sections, C-sections and the so called hat-sections studied and described in the literature, are used mainly as purlins or truss components. A new profile GEB was patented for the use for primary load-bearing member in fabricated steel frames. According to the code [1] every novel cross section should be tested to assign the deformation shape and bearing capacity. The paper deals with the numerical and experimental research of bearing capacity of cold formed GEB profiles. The deformation shape and limit load was obtained from bending tests. The GEB cross section bearing capacity was also determined according to codes [1, 2]. Jedną z najważniejszych zalet lekkich konstrukcji metalowych, wytwarzanych z kształtowników giętych na zimno, jest ich mała masa, dlatego też, producenci coraz częściej wykorzystują możliwości profili giętych do wytwarzania typowych konstrukcji halowych w budownictwie systemowym. Proces gięcia na zimno, pozwala na formowanie różnego rodzaju przekrojów poprzecznych, które mogą być wykorzystywane jako elementy konstrukcji. Typowe kształty elementów. tzn.     Maintenance cost of the equipment is one of the most important portions of the operating expenditures in mines; therefore, any change in the equipment productivity can lead to major changes in the unit cost of the production. This clearly shows the importance and necessity of using novel maintenance methods instead of traditional approaches, in order to reach the minimum sudden occurrence of the equipment failure. For instance, the tires are costly components in maintenance which should be regularly inspected and replaced among different axles. The paper investigates the current condition of equipment tires at Sungun Copper Mine and uses neural networks to estimate the wear of the tires. The Input parameters of the network composed of initial tread depth, time of inspection and consumed tread depth by the time of inspection. The output of the network is considered as the residual service time ratio of the tires. The network trained by the feed-forward back propagation learning algorithm. Results revealed a good coincidence between the real and estimated values as 96.6% of correlation coefficient. Hence, better decisions could be made about the tires to reduce the sudden failures and equipment breakdowns. Koszty użytkowania sprzętu stanowią jedną z najpoważniejszych pozycji w zestawieniu kosztów eksploatacyjnych kopalni, dlatego też każda poprawa wydajności sprzętu powoduje w efekcie zmianę jednostkowego kosztu produkcji.     In recent years, water jet cutting technology has been being used more and more often, in various domains of human activity. Its numerous applications include cutting different materials - among them, rock materials. The present paper discusses the results of the research that aimed at determining - in a quantitative manner - the way in which the water jet cutting parameters (such as the traverse speed of the head, and the distance between the high-pressure inlet of the water jet and the cut material) influence the quality of the processed surface. Additionally, the impact of these parameters on the surface of various materials was investigated. The materials used were three granites differing with respect to the size of grains. In the course of the research, the standard parameters defined by the ISO norms were analyzed. It was also proposed that variograms be used to analyze the quality of the cut surface. Jest ona wykorzystywana do obróbki różnorodnych materiałów, również materiałów skalnych. W ramach badań analizowano trzy granity różniące się m.in. wielkościami ziarn, które były przecinane przy różnych prędkościach przesuwu głowicy z wlotem strumienia wodnego. Analizowano standardowe parametry zdefiniowane w normach ISO jak również zaproponowano wykorzystanie wariogramów do analizy jakości wyciętej powierzchni. W pracy opisano w sposób ilościowy zmiany jakości powierzchni skał ciętych strumieniem wodnym ze ścierniwem w zależności od prędkości przesuwu głowicy, jak również w zależności od odległości przecinanego fragmentu powierzchni od wlotu strumienia wodnego do materiału.     In the paper the contaminants that may be generated in Underground Coal Gasification (UCG) process were listed and include mainly mono- and polycyclic aromatic hydrocarbons, phenols, heavy metals, cyanides, ammonium, chloride and sulphate. Granulated activated carbon (GAC) and zeolite, and granulated activated carbon and scrap iron were applied in the first and second test respectively. For these materials the hydro geological parameters called reactive material parameters were determined and discussed. The results of the experiments showed that GAC seems to be the most effective material for phenols, BTX, PAH, cyanides and slightly lowers ammonia removal, while zeolites and scrap iron removed free cyanide, ammonia and heavy metals respectively. Podziemne Zgazowanie Węgla (PZW) jest alternatywną metodą pozyskiwania energii z węgla. Jest to zespół przemian termicznych i chemicznych przebiegających bezpośrednio w złożu węgla, zachodzących pomiędzy substancją organiczną a czynnikiem zgazowującym, jakim może być powietrze, tlen, para wodna, dwutlenek węgla. Poza wieloma zaletami metoda ta niesie za sobą także wiele zagrożeń, które były rozważane w ramach projektu HUGE 2 (nr RFCR-CT-2011-00002). Jednym z nich jest zagrożenie środowiska wód podziemnych produktami PZW, do których należą wielopierścieniowe węglowodory aromatyczne, BTX, fenole, metale ciężkie, cyjanki, jony amonowe, chlorki i siarczany. W celu zminimalizowania tego zagrożenia w pracy rozważono zastosowanie w obszarze reaktora PZW technologii Przepuszczalnej Bariery Reaktywnej (PRB). W technologii tej zanieczyszczenia usuwane są in-situ poprzez przepływ wód przez odpowiednio dobrany materiał reaktywny. Istotną dla procesu badawczego wskazówkę stanowiła informacja, o ostrokrawędzistości ziarn minerałów regolitu księżycowego (Rybus T., 2009), (Sibille i in., 2006). W artykule, w formie graficznej oraz zestawień tabelarycznych, zestawiono wartości najistotniejszych, dla porównań dwóch gruntów, wielkości takich jak: skład granulometryczny (rys. 1), charakterystyki krzywych uziarnienia (tab. 1), wybrane właściwości fizyczne, np. gęstość nasypowa, barwa (pkt. 2), parametry mechaniczne, takie jak wytrzymałość na ścinanie, kąt tarcia wewnętrznego i kohezja (rys. 2 i 3, tab. 2). Istotnym osiągnięciem prowadzonych badań było uzyskanie polskiego zamiennika gruntu księżycowego, którego jednostkowy koszt wytworzenia jest wielokrotnie mniejszy od jednostkowego kosztu zakupu gruntu wzorcowego. Opracowany, polski analog gruntu księżycowego AGK-2010 (zgłoszony do Urzędu Patentowego w 2011 r.) został z powodzeniem wdrożony w testach penetratora KRET w Centrum Badań Kosmicznych Polskiej Akademii Nauk w Warszawie.

    minerałów miedzionośnych oraz rodzajem ich skupień i wpryśnięć. Niezwykle istotne jest właściwe rozpoznanie rudy pod względem petrograficzno-mineralogicznym. Znajomość właściwości przerabianej rudy pozwala na pełniejsze jej wykorzystanie poprzez właściwe prowadzenie i sterowanie procesami, dobór ich parametrów takich m.in. jak: czas mielenia, parametry klasyfikacji, rodzaj odczynników flotacyjnych, czas flotacji. W artykule przedstawiono wyniki przeprowadzonych analiz mikroskopowych, analizy obrazów zdjęć skaningowych oraz procedury wykorzystania otrzymanych informacji do identyfikacji typów rud (zastosowanie sieci neuronowych do rozpoznawania składów litologicznych). W badaniach wykorzystano rudę miedzi, pochodzącą z jednego z zakładów górniczych KGHM Polska Miedź S.A. Pobrane próbki rudy reprezentowały każdy z trzech typów litologicznych występujących w krajowych złożach: węglanową, łupkową oraz piaskowcową. Przeprowadzono ilościową analizę mineralogiczno-petrograficzną przy pomocy mikroskopu optycznego w świetle odbitym, a wyniki przedstawiono w tabelach 1 i 2. Wyniki te określają ilościowo stopień uwolnienia i zrosty dla jednego rodzaju minerału. Pokazano także przykładowe zdjęcia mikroskopowe zgładów poszczególnych typów litologicznych rud oraz próbek proszkowych tych typów (rys. 1 i 2). W kolejnym etapie badań wykorzystano skaningowy mikroskop elektronowy. Zdjęcia morfologii próbek uzyskane z mikroskopu elektronowego (obrazy SEM) były przedmiotem komputerowej analizy obrazu, umożliwiającej mikrostrukturalną klasyfikację ilościową typów rud. Analizowano wszystkie próbki poszczególnych typów litologicznych rudy miedzi: węglanowej, łupkowej i piaskowcowej oraz mieszanki tych typów w trzech klasach ziarnowych: 0÷45 μm, 45÷71 μm i 71÷100 μm.     The paper considers some results of creating load-carrying composite systems that have uprated strength, rigidity and safety, and therefore are called geometrically (self-) hardening systems. The optimization mathematic models of structures as discrete mechanical systems withstanding dead load, monotonic or low cyclic static and kinematic actions are proposed. To find limit parameters of these actions the extreme energetic principle is suggested what result in the bilevel mathematic programming problem statement. The limit parameters of load actions are found on the first level of optimization. On the second level the power of the constant load with equilibrium preloading is maximized and/or system cost is minimized. The examples of using the proposed methods are presented and geometrically hardening composite steel-concrete system are taken into account. W pracy przedstawiono sposoby projektowania konstrukcji, które ze względu na swoją geometrię oraz topologię posiadają podwyższoną nośność, sztywność i bezpieczeństwo. Systemy takie nazwano geometrycznie (samo-) wzmacniającymi się. Zaproponowano optymalizacyjne modele matematyczne konstrukcji jako dyskretne systemy mechaniczne będące pod obciążeniem stałym, zmiennym monotoniczne lub niskocyklowym, statycznym lub kinematycznym. Dla znalezienia granicznych parametrów obciążeń wprowadzona została ekstremalna zasada energetyczna, przedstawiona jako problem dwupoziomowego programowania matematycznego. Graniczne parametry obciążeń szukane są na pierwszym poziomie optymalizacji. Na drugim poziomie minimalizowany jest koszt systemu i/lub maksymalizowana jest moc stałego równoważącego obciążenia z dociążeniem. ( The Chemical Company) - Dział Domieszek do Betonu], wpływa korzystnie na parametry reologiczne zaczynu uszczelniającego sporządzanego na osnowie cementu hutniczego CEM III /A 32,5. W przeprowadzanych badaniach zmiennymi były: • rodzaj superplastyfikatora, • rodzaj cieczy zarobowej. W badaniach laboratoryjnych zastosowano następujące superplastyfikatory firmy BASF: • SKY 501, • SKY 503, • SKY 591, • ACE 430, • Glenium 115. Koncentracja superplastyfikatora w zaczynie wynosiła: 0,5% wagowo (w stosunku do masy suchego cementu). Współczynnik wodno-cementowy dla badanych zaczynów uszczelniających wynosił: 0,5. Do sporządzania zaczynów uszczelniających stosowano cement hutniczy klasy CEM III/A 32,5 N-LH/ HSR/NA - Cementownia Małogoszcz, Grupa Lafarge Cement S.A. w Małogoszczy.

  10. Simulations of Moscow megacity heat island with the COSMO-CLM model using two different urban canopy schemes and realistic building parameters, derived from OpenStreetMap data

    In this study we present the application of the regional climate model COSMO-CLM to simulate urban heat island (UHI) phenomenon for Moscow megacity, which is the biggest agglomeration in Europe (with modern population of more than 17 million people). Significant differences of Moscow from the cities of Western Europe are related with much more continental climate with higher diurnal and annual temperature variations, and with specific building features such as its high density and almost total predominance of high-rise and low-rise blocks of flats on the private low-rise houses. Because of these building and climate features, the UHI of Moscow megacity is stronger than UHIs of many other cities of the similar size, with a mean intensity is about 2 °C and maximum intensity reaching up to 13 °C (Lokoschenko, 2014). Such a pronounced UHI together with the existence of an extensive observation network (more than 50 weather and air quality monitoring stations and few microwave temperature profilers) within the city and its surrounding make Moscow an especially interesting place for urban climate researches and good testbed for urban canopy models. In our numerical experiments, regional climate model firstly was adapted for investigated region with aim to improve quality of its simulations of rural areas. Then, to take into account urban canopy effects on thermal regime of the urbanized areas, we used two different versions of COSMO-CLM model. First is coupled with TEB (Town Energy Balance) single layer urban canopy model (Trusilova, 2013), and second is extended with bulk urban canopy scheme TERRA_URB using the Semi-empircal URban-canopY dependency parametriation SURY (Wouters et. al, 2016). Numerical experiments with these two versions of the model were run with spatial resolution about 1 km for several summer and winter months. Parauga virsmas polarizēšanas laukumu var mainīt ar gredzenveida starplikām, ko novieto virs parauga virsmas. Ierīce ir pilnībā datorizēta, kas ļauj sekot koronas izlādes gaitai, to vadīt un reģistrēt rezultātus. Tas savukārt uzlabo NLO polimēru paraugu uzlādēšanas procesu pētījumu kvalitāti un ļauj veiksmīgāk noteikt optimālāko polarizēšanas režīmu. Izmantojot strāvas-sprieguma raksturlīknes un polarizēto polimēru otrās harmonikas mērījumus, var arī uzskatāmi parādīt, kā polarizēšanas ierīces un tās darba režīma parametri ietekmē polarizēto plāno kārtiņu NLO efektivitāti.

    pożaru niektórych warunków panujących w danym miejscu kopalni, takich jak: - skłonność węgla do samozapalania - zawartość tlenu w powietrzu dopływającym do zagrzewającego się węgla. W pracy przedstawiono numeryczną metodę wyznaczania warunków krytycznych samozapalania węgla w zrobach ścian, czyli takich po spełnieniu których może dojść do samozapalenia. Przyjęto, że znajdujący się w zrobach rozkruszony węgiel ma kształt płaskiej warstwy, sąsiadującej od stropu i spągu ze skałami. Rozważania ograniczono do węgli charakteryzujących się niską zawartością wilgoci. Przyjęto prosty model kinetyki utleniania na powierzchni węgla wyrażony równaniem Arrheniusa. Model ten zakłada niezależność szybkości utleniania od ilości tlenu pochłoniętego przez węgiel. Szybkość reakcji zależy jedynie od temperatury , przy czym występujące w równaniu parametry zmieniają się po osiągnięciu przez węgiel temperatury krytycznej. W artykule przedstawiono matematyczny model samozagrzewania warstwy węgla w zrobach opisujący bilans ciepła w węglu oraz bilans tlenu i bilans ciepła w przepływających gazach. Tworzące model układy równań różniczkowych cząstkowych rozwiązywane są metodami numerycznymi. Opracowany program komputerowy umożliwia wykonywanie stosownych obliczeń. W pracy, na przykładzie węgla z pokładu 405 , przedstawiono sposób wyznaczania warunków krytycznych warstwy rozkruszonego węgla: grubości warstwy, zawartości tlenu w przepływających przez warstwę gazach oraz przewodności cieplnej otaczających skał.

Opisano podstawowe zasady metody kompleksowej oceny stanu zagrożenia tąpnięciem, w skład której wchodzi metoda sejsmologiczna bieżącej (pomiarowej) oceny stanu zagrożenia. Od wielu lat, wraz z ciągłym rozwojem bazy aparaturowej i możliwości w zakresie cyfrowej rejestracji sejsmogramów oraz przetwarzania i interpretacji danych pomiarowych wzrasta znaczenie metod sejsmicznych, które są dzisiaj powszechnie stosowane w polskich kopalniach zagrożonych tąpaniami. Ciągła obserwacja zjawisk sejsmicznych indukowanych w trakcie rozwoju procesu eksploatacji pokładów węgla umożliwiła, w oparciu o zgromadzoną bazę danych, opracowywanie nowych kryteriów zagrożenia sejsmicznego oraz zagrożenia tąpnięciem, które winny wyraźnie poprawić efektywność metody sejsmologii górniczej. W artykule przedstawiono nowe kryteria oceny stanu bieżącego zagrożenia tąpaniami zaproponowane do stosowania w polskich kopalniach węgla kamiennego, które prowadzą eksploatację systemem ścianowym. Kryteria te są oparte na wynikach ciągłej rejestracji sejsmologicznej, połączonej z bieżącą analizą zarejestrowanych wstrząsów i obliczaniem wybranych parametrów sejsmologicznych. Parametry te to położenie ognisk wstrząsów w stosunku do wyrobisk eksploatacyjnych, energia sejsmiczna wstrząsów, suma energii sejsmicznej wyzwolona na każde 5m postępu ściany eksploatacyjnej, wartość wagowanego parametru amplitudy prędkości drgań PPVW, moment sejsmiczny M0, indeks energii EI oraz parametr b rozkładu wstrząsów według relacji Gutenberga-Richtera.     pressure values on the basis of the available results of measurements carried out in adjacent nodes, as well as the pressure value calculated on the basis of the heading geometry and the given volumetric flow rate. The present paper discusses the methodology of compensating and balancing the volumetric air flow rates within a network of headings (Chapter 2) and the methodology of determining pressure values (Chapter 3) in the nodes of the network. The developed calculation algorithms - verified by means of sample calculations performed for a selected area of a mine ventilation network - were introduced into the VentGraph software system. The calculation results were presented in tabular form. The Summary section discusses the minuses and pluses of the adopted methodology. Podstawą prognozy procesu przewietrzania jest posługiwanie się zwalidowanym programem komputerowym (Dziurzyński i in., 2011; Pritchard, 2010) oraz poprawnie przygotowaną bazą danych zawierającą parametry opisujące przepływ powietrza i gazów, zgodną z przyjętym modelem matematycznym w programie komputerowym VentGraph (Dziurzyński, 2002). Dysponując bazą danych pomiarowych oraz przyjętym do obliczeń komputerowych i symulacji procesu przewietrzania modelem matematycznym przystępujemy do opracowania modelu numerycznego dla wybranej sieci wyrobisk kopalni. Przygotowanie modelu numerycznego sieci wentylacyjnej danej kopalni wymaga dostarczenia zestawu danych dotyczących struktury sieci i własności fizycznych jej elementów, tj. wyrobisk, wentylatorów, tam, a przy symulacji pożaru dodatkowo wymagane jest podanie parametrów opisujących ognisko pożaru oraz własności skał górotworu. Obecna praktyka postępowania polega na tym, że wykonuje się ręczne pomiary wentylacyjne w wyrobiskach górniczych, a uzyskane wyniki stanowią podstawę do wyznaczenia parametrów fizycznych takich jak: opór aerodynamiczny wyrobiska, gęstość przepływającego powietrza i

W prawidłowo zlikwidowanym szybie podsadzka winna szczelnie wypełniać rurę szybową do poziomu zrębu, a w przypadku wystąpienia procesu jej osiadania - okresowo uzupełniana. W takim przypadku obciążenie wypadkowe obudowy zlikwidowanego szybu można wyrazić wzorami (12), (13). Jednak w zależności od rodzaju zastosowanego materiału podsadzkowego do likwidacji szybu, jak to wynika z prowadzonych badań ich parametry fizyko-mechaniczne mogą ulegać zmianie w czasie w mniejszym lub większym stopniu. W artykule powołano się na badania górniczych materiałów odpadowych pochodzących z robót dołowych i przeróbczych. Zmianę istotnych parametrów, z punktu widzenia obliczanych wartości obciążenia pionowego i poziomego wywieranego przez podsadzkę na obudowę zlikwidowanego szybu przedstawiono na wykresach 3 i 4. Uwzględnienie tych zmian dla przedstawionego przykładu obliczeniowego wykazało wzrost wielkości parcia podsadzki o około 19%. Ponadto w zlikwidowanych szybach, w których występuje dopływ wody zza obudowy koniecznym jest, aby materiał użyty do likwidacji posiadał wymagany współczynnik filtracji, co obrazuje graficzna interpretacja przykładowych wyników obliczeń przedstawiona na Rys. 6. Nawet prawidłowo dobrany materiał zasypowy (z punktu widzenia wymaganej wartości współczynnika filtracji) może ulegać zmianom uziarnienia w wyniku takich czynników jak: swobodny spadek do szybu w czasie likwidacji, długotrwałe narażenie na oddziaływanie wody, wynoszenie drobnych cząstek gruntu wraz z wodą dopływającą zza obudowy (sufozja) itp. Nóż styczno-obrotowy podczas eksploatacji jest elementem będącym bezpośrednio w kontakcie z urabianą calizną. Nóż mocowany jest w uchwycie przyspawanym do organu frezującego będącego częścią maszyny urabiającej. Prawidłowa praca samego noża uzależniona jest od parametrów geometrycznych organu i uchwytu, parametrów kinematycznych maszyny urabiającej oraz organu jak również parametrów materiałowych uchwytu. Dopiero gdy parametry te są poprawnie dobrane oraz zachowane możliwe jest zapewnienie prawidłowych warunków pracy noża. Wtedy też jego jakość ma duży wpływ na realizację procesu skrawania, a przede wszystkim na energochłonność oraz trwałość. Zagadnienia teoretyczne związane z procesem frezowania nożami styczno-obrotowymi są podstawą do opracowania metody oceny ich jakości. Jednak w pierwszej kolejności konieczne jest sprecyzowanie odpowiednich założeń, które przedmiotowa metoda musi spełniać: • możliwość porównywania jakości wykonania noży, • możliwość kontroli utrzymania jakości dostarczanych noży, • możliwość zastosowania do prawa zamówień publicznych, • parametryczność pozwalająca na dostosowanie do różnych warunków pracy. Uwzględniając istotne z punktu widzenia prawidłowej pracy noży styczno-obrotowych czynniki określono następujące elementy oceny jakości noży: • pomiar parametrów geometrycznych całego noża oraz samego węglika spiekanego, • badania parametrów materiałowych korpusu noża, • badania parametrów materiałowych ostrza, • pomiar twardości części roboczej i chwytowej korpusu, • pomiar twardości ostrza, • określenie szybkości zużycia noża w warunkach laboratoryjnych. Mając powyższe na uwadze zaproponowano przeprowadzenie badań w trzech etapach [1,7]: • pomiar parametrów geometrycznych noży styczno-obrotowych,

Wspomniane parametry hydrogeologiczne są składowymi równania opisującego hydrauliczną oporność ośrodka wodonośnego (12). Uogólniając, parametr B, może być kryterium wykorzystanym przy planowaniu i projektowaniu systemu odwodnienia studziennego, a także bieżącej oceny skuteczności współpracy systemu. Na rysunku 3. przedstawiono ideowo metodykę oznaczania tego parametru, za pomocą testu próbnego pompowania. Dotychczas interpretacja wyników próbnych pompowań koncentrowała się na ocenie technicznych warunków pracy otworów studziennych. W tym celu wykorzystywano współczynnik C występujący we wzorze Jacoba, który stanowił kryterium odbioru studni od wykonawcy. Zdaniem autorów niniejszej pracy wyniki próbnych pompowań należy interpretować w szerszym ujęciu, zwracając uwagę na parametr B, opisujący oporność hydrauliczną przy przepływie laminarnym wody w ośrodku wodonośnym. Analiza rozkładu przestrzennego zmian tego parametru pozwala na racjonalizację systemu odwodnienia studziennego w tym ocenę niezbędnych wyprzedzeń w zakresie wykonawstwa studni. W praktyce, ocenę geohydraulicznych warunków prowadzonego odwodnienia przeprowadzono przy wykorzystaniu zmodyfikowanej metody próbnych pompowań wielostopniowych opracowanych przez Klicha (1997). W pracy niniejszej przedstawiono wyniki próbnych pompowań w 27 studniach pracujących w kopalni odkrywkowej węgla brunatnego. Na rysunkach 5-9 przedstawiono wyniki oceny parametru B dla podstawowych kompleksów wodonośnych, dla których wykonano próbne pompowania badawcze. Podział wykresów wynika z przeznaczenia studni, co odpowiada podziałowi przedstawionemu na rysunku 4. Uzyskane wyniki pozwalają na sformułowanie wstępnych uogólnień. Uzyskane niższe wartości adsorpcji dla materiałów po modyfikacji niż dla proszku surowego potwierdzają, że materiały posiadają charakter hydrofobowy, co pozwala stwierdzić, że obie proponowane metody hydrofobizacji są użyteczne do produkcji wodoodpornego pyłu wapiennego. Analizę termiczną pyłów wapiennych wykonano w celu porównania efektu cieplnego przemian zachodzących podczas ogrzewania pyłów modyfikowanych z efektem cieplnym zachodzącym podczas ogrzewania surowego pyłu wapiennego. W trakcie badań użyto termowagę firmy TA Instruments 2960 SDT ze sprzężonym spektrometrem masowym firmy Balzers TermoStar 300. Otrzymane wynikiprzedstawiono na Rys. 2. Linie ciągłe na Rys. 2a i 2b przedstawiają przebieg krzywych TG, DTG oraz DTA dla pyłu surowego. Linie przerywane zostały otrzymane dla pyłu modyfikowanego kwasem stearynowym (Rys. 2a) oraz preparatem SARSIL® H-15 (Rys. 2b). Na Rys. 3 zobrazowano wyniki analizy zawartości tlenku węgla IV w składzie gazów otrzymywanych podczas rozkładu termicznego pyłu wapiennego (EGA). Krzywa otrzymana dla pyłu modyfikowanego kwasem stearynowym w zasadzie pokrywa się z krzywą otrzymaną dla pyłu surowego. Parametry uzyskane z użyciem aparatu Powder Characteristics Tester umożliwiły scharakteryzowanie pyłów wapiennych nie tylko pod kątem ich właściwości hydrofobowych, ale także właściwości kohezyjnych i przepływowych.

Zasadniczym problemem jest także wybór jednostki populacji generalnej (ziarno, jednostka objętości lub masy), co może decydować o kierunkach charakteryzowania wielowymiarowych powiązań cech wektora X. Takimi kierunkami charakteryzowania mogą być: - wielowymiarowe rozkłady wektora losowego X wraz ze wszystkimi konsekwencjami metody (Lyman, 1993; Niedoba, 2009; 2011; Olejnik et al., 2010; Niedoba i Surowiak, 2012); - wielowymiarowe równania regresji wraz z analizą macierzy współczynników korelacji liniowej oraz korelacji cząstkowej (Niedoba, 2013b); - analiza czynnikowa (Tumidajski, 1997; Tumidajski and Saramak, 2009); - metody wielowymiarowej wizualizacji danych. W artykule zastosowano nowoczesną metodę wizualizacji wielowymiarowych danych - metodę tzw. map odniesienia (z ang. relevance maps). Aby zastosować ww. metodę przeprowadzono doświadczenia na trzech typach węgla, pobranych z trzech kopalni węgla kamiennego, zlokalizowanych w Górnośląskim Okręgu Przemysłowym. Były to węgle typu 31, 34.2 i 35, według polskiej klasyfikacji węgli. Każdą z pobranych prób poddano rozdziałowi na klasy ziarnowe a następnie każdą z klas ziarnowych rozdzielono na frakcje densymetryczne za pomocą rozdziału w roztworze chlorku cynku. Tak otrzymane klaso-frakcje przebadano chemiczne ze względu na wybrane parametry jakościowe węgla. Były to takie cechy jak: ciepło spalania, zawartość popiołu, zawartość siarki, zawartość substancji lotnych oraz miąższość materiału. Otrzymano w ten sposób zestaw siedmiu danych dla każdej klasy ziarnowej i każdego typu węgla. Stanowił on swoisty siedmiowymiarowy zbiór, który postanowiono zobrazować za pomocą techniki wizualizacji bazującej na tzw. mapach odniesienia. W metodzie map odniesienia na płaszczyźnie służącej do wizualizacji danych zostają rozmieszczone specjalne punkty zwane punktami odniesienia, reprezentujące poszczeg

Funkcję ładowania, realizowaną normalnie przez płaty, przejmują ładowarki. Rozdzielenie procesu ładowania od procesu frezowania jest jedną z najważniejszych zalet prezentowanego rozwiązania. Podczas pracy w ścianie parametry kinematyczne kombajnu mogą być zwiększane bez ryzyka wystąpienia problemów z ładowaniem na przenośnik, co obok możliwości ruchowych kombajnisty było głównym czynnikiem hamującym wzrost wydobycia. Załadunek urobku na przenośnik odbywa się za pomocą ładowarki, która po zmianie kierunku urabiania składa się, natomiast druga ustawiana jest w pozycji roboczej (Krauze, 2010). Kombajn porusza się tradycyjnie po rynnie przenośnika, jednak przy zastosowaniu cięgnowego systemu posuwu, który zrealizowany jest za pomocą napędów znajdujących się w chodnikach. Rozwiązanie takie umożliwia znaczne zmniejszenie gabarytów kombajnu dzięki usunięciu ciągników z kadłuba. Należy zaznaczyć, że w prezentowanym rozwiązaniu zakłada się zastosowanie pełnej automatyzacja pracy kombajnu i pozostałych maszyn ścianowych, co pozwala na urabianie calizny bez obecności załogi bezpośrednio w wyrobisku. Ma to duże znaczenie dla możliwości zwiększenia prędkości posuwu, szczególnie przy zakresie wysokości 1.0 m÷1.6 m. Przy wykorzystaniu znanych, z rozwiązań strugowych, systemów automatyki, sterowania i diagnostyki wprowadzenie automatyzacji procesu nie będzie stanowiło znaczącego problemu. W proponowanym rozwiązaniu kombajn ma możliwość wyjechania do chodnika na tyle, aby możliwe było zrealizowanie przekładki jak w przypadku strugów. Uzyskuje się wtedy krótki czas przekładki, który w skrajnym przypadku może zmieścić się w tzw. czasie organizacyjnym ściany. Ponadto urabianie odbywa się pełnym zabiorem na całej długości

Im większe wydłużenie względne podziałki tym większe wartości osiągają parametry opisujące położenie ogniw w gniazdach koła łańcuchowego (ɛ, u oraz αu). Ze względu na powtarzalność położenia ogniw w gniazdach bębna łańcuchowego o liczbie zębów z podczas nabiegania łańcucha następuje cykliczne obciążanie kolejnych den gniazd, flanek zębów i ogniw w czasie obrotu bębna łańcuchowego o kąt podziałowy φ = 2π/z. W zakresie obrotu bębna łańcuchowego o kąt podziałowy wyróżniono trzy przedziały charakteryzujące się odmiennym sposobem obciążenia elementów bębna łańcuchowego (rys. 2÷4). Poślizg torusa przedniego ogniwa poziomego na dnie gniazda ma miejsce w pierwszym przedziale obciążenia bębna o kąt podziałowy. W przedziale tym wyróżnić można w przegubie przednim dwie fazy: toczenia i poślizgu ogniw. Wyznaczono drogę tarcia oraz pracę tarcia na dnie gniazda podczas toczenia się ogniw oraz podczas poślizgu ogniw w przegubie przednim (zależności 5÷17). Wykorzystując te zależności wyznaczono drogę tarcia i pracę tarcia na dnie gniazda bębna łańcuchowego o liczbie zębów z = 7, współdziałającego z łańcuchem wielkości 34 ×126 mm o podziałkach ogniw wydłużonych o Δp/p = 0,5% i Δp/p = 3,0%. Pracę tarcia na dnie gniazda Ag wyznaczono całkując numerycznie iloczyn drogi tarcia i odpowiedniej wartości siły R w punkcie styku ogniwa z dnem gniazda. Z powodu względnego określenie siły w punkcie styku w stosunku do wartości siły nabiegającej R/SH również pracę tarcia wyznaczono jako względną w stosunku do siły nabiegającej jako Ag /SH. Względną pracę tarcia na dnie gniazda wyznaczono w funkcji kąta obrotu bębna dla różnych wartości współczynnika tarcia na dnia gniazda (rys. 8÷9).     :1998 określone zostały wymagania podsadzki zestalanej oraz dla mieszaniny do doszczelniania zrobów. (tab. 5), tylko dla części badań wskazane są konkretne wymagania ilościowe. Wyniki badań wymywalność zanieczyszczeń chemicznych oraz zawartości radionuklidów zostały omówione wcześniej. Właściwości zawiesin w stanie płynnym zestawiono w tabeli 6. oraz na rys. 7, 8 i 9., a parametry reologiczne wg modelu Binghama na rys. 10÷13. Wymagania w zakresie właściwości zestalonych zawiesin tj. wytrzymałości na jednoosiowe ściskanie oraz rozmakalności zostały ściśle określone dla podsadzki zestalanej jak i mieszaniny do doszczelniania zrobów (tab. 5). Zawiesiny przygotowane na bazie odpadów ze zgazowania węgla nie wykazują właściwości wiążących. Zarówno ze względu na wytrzymałość na jednoosiowe ściskanie (rys. 14) jak i rozmakalność (rys. 15) kwalifikują się jedynie, jako mieszaniny do doszczelniania zrobów. Przeprowadzone badania wstępnie potwierdziły możliwość zastosowania ubocznych produktów zgazowania pochodzących z instalacji bazujących na reaktorach dyspersyjnych, w technologiach górniczych charakterystycznych dla polskiego górnictwa podziemnego. Dla ostatecznego potwierdzenia konieczne są oczywiście badania dokładnie tych odpadów, które miałyby być stosowane w konkretnych kopalniach. Przyszłe wykorzystanie zastosowania odpadów z procesu zgazowania węgla, jako materiałów podsadzkowych zależy od wielu czynników, do których zaliczyć należą: dalszy rozwój technologii podsadzki hydraulicznej w górnictwie polskim, a także właściwości i dostępność tego typu materiałów powstałych ze zgazowania polskich węgli i/oraz w instalacjach na terenie Polski.
